@lucidmed3343 hi, I was wondering if you have any advice about fluid around the heart. My daughter was recently diagnosed with another autoimmune, but this time, it's positive for half lupus and half not. I'm wondering if the air conditioner that's sitting in her bedroom she uses every day might be part of what's causing some issues? She has a cough etc. We've had it for several years and it comes from a home that had mold and asbestos in it. The last time which has been a couple months now that she had IVIG. They were only able to keep it in for about 5 minutes so they tried to switch veins and couldn't get another vein after that. I am also wondering if the IVIG could be part of what was causing the fluid around her heart they called it' pericarditis. They are now starting her on Benlysta.

I was given multiple doses of IVIG back in 1995 when I developed Polymyositis that wasn’t responding to any immune suppressive drugs…the next day after the infusion was the first time in 3 months all the swelling in my body went down…downside ..absolutely horrific migraines, vomiting, fever they called “clinical menengitis” for 72 hours so bad I wished I never gotten it..this side effect was reduced when they slowed the flow of the IVIG during my future infusions..

So happy I discovered your channel. I'm diagnosed with PN/SFN and had ny first in fusion Friday which went well. No side effects til Sunday. Currently have a migraine that feels like a hangover and flu like symptoms, along with photophobia which is made worse due to the the snow reflecting the sun. So today was kinda tough, but trying to take it easy and suck it up. Also trying to add some turmeric and ginger supplements for inflammation too

Hi psyko! (Writing that gave me pause for a sec lol) I'm really glad you're enjoying the channel. Everything you're experiencing is normal. The delayed side effects are absolutely a thing. I'll remind you to hydrate, hydrate, hydrate. Next infusion...start increasing your water intake 48 hours prior to visit. I'll also reassure you that normally side effects improve with each cycle so you shouldn't have this experience every time. Please continue to let me know how you're doing and how the infusions are going. Danielle

I'm getting it next Tuesday. I'm having surgery in 10 days and my platelet count is 43. I've been given prednisone for this the last 2 surgeries with poor outcome. I had platelets given during my gallbladder removal and that worked well. I wonder why my hematologist decided to go IVIG route for me.
@lucidmed3343
@lucidmed3343 Жыл бұрын
Hi Liz!! My guess is that you have chronic immune thrombocytopenia purpura (ITP) where your body is attacking your platelet production. By giving IVIG prior to surgery it will help with this. I hope you do well and have a speedy recovery! Danielle

I would love to hear your opinion about being diagnosed with something that will require getting IV theorapy. I mean for the rest my life. -- damage to your veins, etc. I just started getting home home IVIG and though I don't mind getting IV, I fear the long term effects of IVIG on my body.
@lucidmed3343
@lucidmed3343 Жыл бұрын
Hi Greg! Hope you've been doing well. So with long term infusion there is always a point where veins will become less pliable and begin to show evidence of repeated use...when that exactly happens is reliant on multiple things such as various patient- and practitioner-related factors: age, obesity, history of chemotherapy, vein characteristics of the patient, and the clinical experience of the practitioners. One thing I will discuss with my patients is if I feel we're reaching that point where my ability to gain access is becoming increasingly challenging I'll call their MD to discuss possible port insertion. However I'll also take their duration of therapy and the specific product I'm infusing into consideration. Mediports have their own set of risks and benefits but in the grand scheme of things they're preferable to sclerotic veins if extended therapy is indicated. PICC lines are not a consideration here as they are high maintenance and can only stay in for about 6 weeks max. With IVIG we tend to run this via a regular small catheter but since this is normally done monthly it's not unusual to have a port placed either.. The other option is SCIG. Not sure if you've considered that? D

For gbs should I take ivig again? Otherwise will reduce nerves communication? For example is a lifetime medication for gbs?
@lucidmed3343
@lucidmed3343 11 ай бұрын
Hi duermebaby! Thanks for your question. If you feel you're having a relapse then I'd absolutely seek medical advice. I've had many patients who remained on it for a few months, stopped it to see if they'd be OK, and then restarted it because they felt they were growing weak again. This isn't always the case of course but if you're feeling OK without signs of relapse then there's no reason for you to continue to infuse. Though I'll also point out that a have a few patients who came off IVIG, did well for an extended period and when it did resurface it was bad enough that their MD made the call to maintain them on IVIG. There are many variables involved. Danielle

Good morning! Yes it typically does. IVIG works 2 ways. It quiets down the immune system from attacking it's own tissues and also has anti-inflammatory properties. Danielle

Hi Grace! I hope so too because IVIG is a true lifesaver in the case of immunodeficiency and autoimmune disease. It can be an uphill battle to get it covered by insurance but typically with low Ig it's easily supported by lab work and testing. Hopefully your authorization gets pushed through quickly. If your IVIG is done for five days is it done inpatient at the hospital? And would they need to start a second iv about halfway through treatment because as I understand ivs shouldn’t stay longer than 3 days

Would a course of IVIG be helpful for me. I have long term vaccine injury and chronic nerve pain, and dysesthesia of the scalp (chronic itch, pain, burn). My conditions are considered idiopathic (cause can't be found)
@lucidmed3343
@lucidmed3343 Жыл бұрын
Hi Angeminc! When you say long term vaccine injury, which vaccine? Is this like a long haul Covid issue? We use Ivig for multiple neuropathies and I’ve had many wind up with various neurological conditions post Covid/post Covid vaccine. In certain cases such as myasthenia gravis and acute multiple sclerosis flares we’ll give a cycle or 2 of Ig with good results. Chronic conditions may necessitate continued therapy. Given you have nerve pain it could possibly help but it’s difficult for me to say with any certainty unfortunately. I’d say it’s worth asking your doc about it to see if appropriate. Ig is sometimes tough to get approved by insurance and with idiopathic designation it can be an uphill battle all the way but it can’t hurt to ask. Danielle

How much does it cost.
@lucidmed3343
@lucidmed3343 Жыл бұрын
Hi Lyn, I believe I had addressed this in the IVIG brand video but that question is nearly impossible to answer with any amount of accuracy because insurance coverage is different from provider to provider and policy to policy. It depends on where you are with deductible, copay, dose your doctor has ordered, and whether you're a Medicare recipient or are commercially insured. On average it can be very expensive (several thousands of dollars for a single dose) but again depending on insurance coverage you could have 0 copay as well. The best way to approach this is to speak with either your insurance provider or specialty pharmacy that's connected to your insurance company and ask them to provide a list of formulary brands and based on your policy if it's covered and what tier it's listed under. I had listed the cost per 10 gram vial in the video referenced above. Let me know what you find out :) Danielle

Hi Melissa! Thanks for commenting :). 25 years is a long time so I'm guessing it's working. Very happy to hear that. I'm guessing you're doing 3 days every month? I've had patient's do as much as 80 grams in 1 day and they did OK but that took a few years to work up to. Normally we do 10gm/hour but that's not a hard and fast rule. Judging by your side effects doing it all in 1 day would be quite rough for you. The time commitment sucks though. If you've been on it for that long I'm wondering whether a different brand might change your experience. Certain brands just cause more adverse events and there's a couple which pretty much everyone does well with. Danielle
