After the whole ordeal on the Blessed Isles it was transformed into the Shadow Isles, and like how Kalista woke up as the spirit of vengeance Hecarim rode up fused to his horse in the way he is in game. Also i'm pretty sure the king Kalista served was the king in the name of the item "Blade of the Ruined King".
