Hello. Yes, only a J-1 visa sponsor can issue a DS-2019 form. Perhaps they are asking if you had a DS-2019 form from a previous J-1 visa program.

Hi, I have a question. I had filled the DS 160 saying ‘Trainee’ (with the SEVIS ID and all that was provided by my program online). But I just received the DS 2019 in hand and it mentions the category as ‘Alien physician’. Will this discrepancy be a problem? Should I fill out another DS160 and pay the fee again? Thanks in advance.

Yes, this is a major discrepancy and you should edit your current DS-160.
