One of the best results you've had. I think by and large the primary reasons this result turned out so well, are: 1. Minimal maxillary advancement (rotated), you can see the top of the maxilla where the LeFort is made, is moved forward very minimally. For class 2 I think this is often ideal. 2. Orientation is okay, maybe slightly too downwards. Even though orientation may be too downwards, the flaring of his incisors forced the planning to be advanced less, and A/B plane greater. I think orientation should be more precise to prevent variability in results. I also think it would be nice to be able to reduce the flaring and allow for more advancement at the B point, but of course this is a complicated matter. It seems that genioplasty has the geniohyoid muscle attachments whereas B point has the genioglossus.

@@Joseph-eq5od You would have to basically just not operate on the upper jaw at all. There is very minimal movement here.

You seem very knowledgable of this topic. I am trying to understand how decompensation of incisors inclination affects the whole picture. - Could they extract only the lower premolars to create a bigger overjet or do they need to extract both upper and lower because it would otherwise create big discrepancies in the bite? - If they would extract upper premolars and then advance the maxilla to that position again, the lip shape and position would be very similar if not the same, right? - For this case, would you do any decompensation or do you think they did correctly by leaving the incisors slightly flared? I dont know if it would be worth it to walk around with an even worse bite and profile for a year or so. - When tongue is rested on the roof of the mouth, how much space should there be between the tip of the tongue and upper incisors? Should it be where the palate meets the incisors or more forward/ behind?

I think what separates the good and bad results is the cheekbones and eye area of the patients beforehand, if you cover his mouth and jaw in his before he looks good

After surgery, the repositioning on the bones might have an impact of the body posture. Specially in cases where there was an assymetry that caused tension in the TMJ.
