It’s an initial appointment so I probably won’t get the injection at this one. My cartilage damage is under my patella. In terms of biomechanics (I think that’s the right word), what cardio workouts are best for knee cap damage. Is the elliptical okay? I ask because I went on it a couple of times recently and did have some pain for a day or two afterword (the pain was bearable). I’m generally also wanting to stay in good cardiovascular health. Understanding every patient is different , I have 2 important questions: in general, 1. what does your experience show to be the best frequency of intervals of shots? and 2. what is the best time period to spin the blood for--or what is the most appropriate amount of the concentration of plasma? This video is very informative, I did 1 PRP injection on my left knee and it was very painful and my leg froze I couldn't walk it was horrible but after a few hours there was no pain, just discomfort for about 4days.and I have 2more to do, not looking forward to it but it's necessary.

My knee has taken alot of abuse. 5 surgeries in all. Acl, mcl, pcl torn. Meniscus torn twice. Infection after operation, septic arthritis. Just before covid, found out hair line can be seen on cartilage. Pain as hell. Few months back, did prp on my knee. Extracted 5 tubes of blood for spinning in centrifuge. Knee feels so much better now. Am only in my 30+.gonna continue for maintenance purposes.

This channel is great. Thanks for all you do. I know you can't give medical advice here. I have what I think is a rare tendinopathy, it's bilateral calcific insertional triceps tendinopathy. The calcifications are visible on x-ray and MRI confirms fluid in the tendons. The problem goes back several years, I had taken a long layoff from the gym to resolve the left side. When I finally got back into it I only had minor pain. I built up slowly only doing pushups. Eventually starting triceps extensions with only 3 lbs. After many months I graduated up to 10 lbs and out of nowhere was getting pain on my right side, near full extension of the elbow, accompanied by tenderness on the lateral side of the olecranon. I stopped doing the extensions but continued with pushups because there was no real pain. That was 10 months ago. In an attempt to finally resolve this, I laid off pressing exercises altogether for 4 weeks, then did some very light pressing I had no pain but the elbow was irritated a few hours later. So, I seem to be worse off for taking the layoff. I'm scheduled for a PRP shot as I've had 2 opinions that think that's a reasonable option. Neither opinion thinks the calcifications are the issue or should be dealt with directly. So my question is if PRP has any research for insertional tendinopathies? Also, should the calcifications be dealt with could they be the source of the pain?

Pretty much all the PRP research on tendons is insertional or origin. There are no (that I can think of) studies looking specifically at triceps tendinopathy, but anecdotally, yes it works. Good luck!

I am a 53 year old with severe OA in my subcalcaneal and knee joint. I had 2 PRP in the subcalcaneal joint, 1 PRP on the left knee, and 2 PRP on my right. I have been pain free for 5 years now. Just wish I could find a clinic to inject PRP into my hip. My current regenerative clinic does not have ultrasound or fluoro guidance imaging.

@@Cybernetic263 I noticed improvements about the 6 week mark. Ask your doctor what and when to start strengthening exercises. That part is super important. No squats or lunges. Buy a foam exercise roll, sit on the ground with the roll under the pocket of your back knee, isolating your thighs & knee, and just extend straightening your knee. No weights, just plain foot lower leg extensions. Eventually ankle weights or a large bag of ice in a pillow case if you don’t have weights, just for the mild resistance. Some people see results in 4 weeks, some 12. It just depends on the extent of knee damage. One knee I had a second injection after seeing no results after 3 months. That second one worked and has not given me problems since my 2019 procedures.

I just had PRP injections in my back last Friday and it seems like the pain is starting to get better. I have 3 bulging discs and a herniated disc with majority of the pain around L2/L3 and L3/L4. Just out of curiosity, did you have bulging or herniated discs and how are you now? Did you have to receive additional PRP injections?

I have been dealing with constant pain in my lower back on my left side as well as muscle inflammation over the past 20 years, but it got worst this past year. MRIs showed a couple of herniated discs and an annular tear. PT and acupuncture did not seem to help relieve the pain at all. I even had an MIS discectomy, which did not help and may have been made the pain slightly worse. I have started doing softwave therapy, but not sure how much that is helping me so far, but I heard very good things about PRP, so I am still hopeful and optimistic. They even say that combining softwave with PRP can help the body heal and recover faster. Even if PRP/softwave does not help, I am considering DiscSeel as my next option. I am hoping to avoid any additional surgery, such as spinal fusion, at this point.

As for me, I came to the realization that the intense pain I am experiencing is at the L2/L3 level, where I have a slight disc bulge with thecal sac indentation and where I did not have surgery. I am still doing softwave therapy and have my intradiscal PRP injections next week. I am hoping that the combination of both treatments will provide significant pain relief.

@@mikea8294 Yes. Actually, from the other source of rejuvenation therapy that has actually been shown to work: NAD+ intravenous therapy. Boy does it ever help my back pain. I just can't afford to get it enough. It's $1000/six-hour session. Soon I hope to be able to, as I could before: about once every three months. It's currently been a year.

I was disappointed with my PRP treatments, inflammation did dissipate but the OA pain in my knee just came back. 3 months later second treatment and knee pain was worse. I personally had more success with Haurlic Acid injections, specifically Monovisc. I was so optimistic about the about the procedure. Good luck.

Thanks for your comment and sharing your experience. There's a lot of research (will make a vide on this very soon) on the different types of PRP, specifically looking at platelet counts. It'd be interesting to learn how many platelets you had in your PRP injection (can probably get a rough estimate by how much blood they drew) and correlate that with outcomes. HA injections are still a good option!

@@JeffreyPengMD I am 54, very fit. I ran marathons, mud runs, and other competitive type races. I' have done weight training and calisthenics my entire life. I eat a healthy diet. My draw was about 5ml of blood both times. My platelets were over 350 based of my labs. I also was a PT patient for the same issue for over two yrs before trying the gels injections. I tried PRP because of the current information available, as I stated my results were not as positive. Thank you for the response.

Hi, I was just reading that you said they obtained about 5ml of your blood. I'm pretty sure that amount is no where near the required. 50cc is the goal! You need a platelet count of at least 10 billion or more.

I just watched this again and you are very convincing. I would love to try this therapy but have the following concerns. First of all my Uncle had the treatment on his hip and it did not work at all, I am not sure if he followed the instructions perfectly so am not sure as why it did not work. I also visited a regenexx office in Larkspur and they took xrays and told me it would not work. I assume because of lack of cartilage. Does this make sense to you and how do you diagnose the possibility of it working? Thank You.

Local anesthetics such as lidocaine has been shown to damage platelets. However, ropivacaine, is much less damaging to platelets and a small amount can be used to make these procedures much more tolerable!

Thanks for your question, these are all "in general" and of course needs to be individualized. There are less studies for ligaments and muscles. We lump them into the tendon category of most of the time needing just 1.

Thanks for the content , after Prp injection in my knee , it becomes swollen , with pain 12 hours after injection which lasts for three days , also i felt sometimes locking in my knees don’t know why !! May be because of the volume !! Anyway after the fourth day my knee movement returns back to normal without locking . Any explanation for these symptoms !! Is it indication that the injection in right place ?? Can it help for meniscus tear ? Theoretically it has huge amount of growth factors , can this induce cartilage or chondrocyte to regenerate the cartilage ?? Thanks

Hello Mr Peng ,thanks for the informative video.I got a PRP treatment on my knee for osteoarthritis and a week after it swallowed and got very painful no major incident I just used a strong pain relief cream with camphor ,chondrotine and collagen is that could be the reason of the sudden swallow and strong pain ?I m 10 days from the shot now the swallowing is slowly less so is the pain but still very painful can t walk properly ...will this affect the platelets ? have a good day thank you .

Excellent video, and appreciate the detail. I continue to get the "no data" response from doctors I'm seeing (and doctor friends) re: PRP, and it seems by your analysis there is a quite a bit of data. I'm considering it for a pesky, ongoing (+8 months) either brachioradialis strain or bicep tendonitis (two docs, two assessments). It continues to be irritated by select weight-lifting and other bicep pulling motions.

This is the first video I've come across that really gets to the questions I have. Thank you for cutting the BS and just getting to the point. One question I do still have though, I have two completely torn ligaments in my left ankle (ATFL and CFL) so my ankle is pretty unstable. I'm running fine on it but I easily roll my ankle on trail runs even with a brace. This happened in Feb 2019. Does the 3.5 year gap matter or would I still be a candidate for this treatment? Thanks!

Unfortunately PRP has not been studied for ligament tears. We do it a lot for UCL problems in the elbow, but results have been mixed. Using PRP for ankle ligament issues would still be considered highly experimental, regardless of when the injury occurred. Best to discuss with your health care provider. Good luck!

@@JeffreyPengMD I'm having a consultation with a specialist regarding PRP or other injections for relief in my elbow where I had a UCL tear a year ago. After the initial tear, 3 months of physical therapy helped me regain use of my arm, and then a few months after that it felt pretty good. But its been degrading a bit the last few months. Are there alternatives to PRP for the UCL for pain relief and recovery?

Great information. 4 years ago I had 3 doctors who told me I needed surgery for the rotatory cuff. I found and alternative Bio energetic Dr. I had various injections (7 sessions) PRP injections for my shoulder and I'm 90 % cured. Now I have a meniscus tear in my right knee. I have app with another Dr that also uses PRP. (the previous had retired) I hope I will have the same results as my shoulder. I know at least 5 people that had knee arthroscopy surgery and was not successful. They are in pain and still going to physiotherapy. Thanks Dr Peng I just subscribe to your excelent videos.

Thanks for your comment. I'm guessing you are saying you had a cortisone injection for your shoulder and are now asking if you can get a PRP injection in your elbow. This is tricky and you'll need to discuss optimal timing with your health care provider. Some doctors will go ahead and do it. Some will wait 1 month. Some will wait 3 months. Unfortunately, there are no studies looking at "best timing". Best of luck

This video is very helpful but the part about preparing for your prp begs a new question. If we rid ourselves of Tylenol, Motrin and eat tons of fruits and vegetables, would that help osteoarthritis in all our joints overall? Or are some joints just stubborn?

Lifestyle has a HUGE impact on osteoarthritis flare ups. But once you have arthritis, modifying your lifestyle will not regrow cartilage and get you a new knee. PRP can help treat the internal environment of the knee to protect the cartilage that is remaining.

I have developed tendinosis in my extensor carpi ulnaris tendon in both of my wrists due to an old Factory job that required repetitive movement and the tendons just didn't heal properly.
