Upon watching this film a second time, I remember that shot showing a bouquet of flowers on a table with a little note asking Sue to 'break a leg' for the big night on New Year's Eve. Gotta say, that's some subtle foreshadowing right there. Good job, movie.

I saw this in the cinema, and it's amazing how much difference the big screen (and more importantly, the big SOUND) makes to this film. In the cinema that rasping of the dude's hands on Sue's outfit made my skin crawl and the scare chord when she first saw her aged finger made me jump, while it sounds almost cartoonish here. Still an interesting film, but the small screen takes away A LOT of its disgusting, visceral, uncomfortable power.

Silly question: If they had respected the balance, does that mean Sue would stay young for as long as Elizabeth was alive (until she died of old age), or does Sue also grow old (until Elizabeth died of old age)? Or does Sue stay young and sexy until Elizabeth dies of old age? But Elizabeth at least gets to enjoy the final weeks of being her younger self? I dunno, so many questions (but it's an illogical fable so maybe nevermind).
and if they had changed after 6 days, would that have any impact of the cycle? or would it just be on spare day for reasons.

Elizabeth was stuck in a rut. If she gave up on The Substance, she'd be stuck with the scars left by her careless behavior. After being Sue, going back to Elizabeth was torture for her. Notice how sedentary she became as Elizabeth, she'd be waiting the whole week to go back to being Sue.

In another youtubers video he read that there was a deleted scene that more clearly showed they knew what the other was doing. But as you said I also noticed each seemed surprised every time they woke up. But the old man totally remembered her in the cafe so maybe that was meant to be definitive proof that their memories are shared.
