He doesn’t seem to understand that we aren’t members of that fictional world idolising a person real to us. We are watching a movie, and we like a well done character in said movie. I think he never did have a visitor. Think about it, the guards all hated him and Arthur literally had nobody who wanted to see him by the end of the film. Plus, the guards would have been suspicious the he wasn't following them to the "visitor" and they would have checked on him. The guards were setting him up to get shanked.

I’ve seen the movie and although that was considered the ending. I have a couple head canons on what the ending actually was. 1. The jokes punchline. When you watch the movie a lot of people thought joker would live, would be free, non guilty, but he’s guilty, still in Arkham, and dies at the end. It’s also a partial rip on the original ending where almost the exact opposite happens. 2. The person who killed him was an insane fan who was in love with the joker, but when Arthur quits being joker he gets upset. Like how obsessed fans would act when they find out they are in a relationship with someone else. And the person who kills Arthur is just a nobody who was a reference to the Nolan movies. To me that makes more since to me and is a better way to look at the ending. Especially since I don’t think it implies much of anything to prove that Arthur is a fake. Those are my opinions/better ways to look at the ending.

It’s sad that we’ll never get a sequel to the joker. But, I guess, just like pacific rim, sometimes it’s best to just leave a franchise at one movie and be happy with it. Would’ve been nice with another the joker movie, though. Maybe one where Bruce Wayne had grown up to become Batman and the movie would center around Arthur’s obsession with Batman as his mere presence stands against Arthur’s beliefs that hope can’t exist in their society. Arthur spends the majority of the movie slowly losing his mind as he gets more and more obsessed with proving that Batman isn’t a martyr of hope, eventually leading to him wanting Batman to kill him just to prove that he’s right. But, as Batman refuses to kill him, Arthur starts committing absolutely heinous crimes, worse than before, in order to get Batman mad enough to kill him. The movie would end with Batman persevering and catching Arthur at the end of the climax, leading to Arthur absolutely losing it and snaps, trying to just kill Batman himself so he won’t stand in his way any longer, only for him to fall from whatever high place they fought at and leading to his death, partly. Arthur manages to survive the fall, but won’t last much longer. In one final attempt, Arthur blames Batman for his death, trying to make the hero think that he’s the reason for Arthur’s death and is just as bad as him. Whether Batman will actually fall for this will be left up to interpretation as the movie ends with Batman standing over Arthur’s dead body as rain falls around them.
