Can restrictive cardiomyopathy and severe RV failure cause Giant a Wave ? Regards

Hi Hassan anything that increases RV resistance to filling can cause giant a waves. Certainly RV failure can do that. I have not seen restrictive CM listed as a cause of giant a waves, but theoretically it should, including when the RV is directly involved in the restrictive process and becomes stiffened and also via pulmonary hypertension from LV involvement.

Where the Hell is the identification of every single wave will help me on myh clinical diagnosis and management especially when this is not a sensitive sign to take intpo consideration ;!! RIDICULOUS that this is still learned

I was fooled by case 22:35 , thought it was a constrictive pericarditis associated with W SIGN :)

Hassan that’s when you know you’re getting good - when you think about W sign in that case ! Remember, being at the bedside makes a huge difference, much easier to identify what’s happening.
