I can't say much since I haven't played Perfect Cherry Blossom myself, but apparently the Netherworld is an actual part of Gensokyo. However, "travel between the two is restricted by a gate above the clouds, sealed by a strong barrier". It is where the souls of the dead go, like a Purgatory in Dante's Divine Comedy. Within the Netherworld lies the Hakugyokurou, a ghost shrine that oversees and organizes the area. Youmu is the shrine's gardener - yes, it actually does need a gardener, since it's a huge shrine hugely famous for its huge gardens - while Yuyuko is the shrine's lady, as well as PCB's boss and main antagonist. After the PCB events, though, Yuyuko and Youmu (and Yukari) are mostly seen as neutral parties instead of antagonists, and will aid you whenever needed if you go to them and ask properly, politely and with good reason. Since Gensokyo is a land of youkai, it's hard for someone to be an... antagonist. Most people are just beat up by an incident-solving heroine, then become friendly.
