Dr. Ebraheim’s educational animated video describing the anatomy and associated injuries of the knee joint.
Disrupted quadriceps
•Patient is unable to actively extend the knee.
The most common cause of ACL ruptures:
•Traumatic force being applied during twisting motion.
•Side stepping or landing from a jump.
Patient complains of:
•Immediate pain
•Knee giving way
•Swelling
Aspiration of the knee
•If aspiration of the knee joint shows evidence of blood within the joint there is 75-80% chance of ACL and meniscal injury.
Lachamn’s test- ACL knee exam
•Knee is flexed at 30 degrees.
•ACL tear of the knee is identified by pulling on the tibia and examining the frontward motion of the lower leg in comparison to the upper leg.
Radiological exam - ACL
•MRI of the knee joint shows bone lesions or bruising associated with tears of the ACL. Injury is found in the typical location; middle of the femoral condyle and posterior part of the tibia laterally.
Posterior cruciate ligament tear (PCL)
•Common cause of injury is a bent knee hitting a dashboard in a car accident.
Tibial Sag Test -PCL knee exam
Quadriceps active test-PCL knee exam
•The examiner stabilizes the leg of the patient and then the patient is asked to actively contract the quadriceps muscle.
•The tibia is seen actively reduced from the posterior subluxed position.
Lachman’s test-PCL knee exam
•Knee is bent 20-30 degrees.
•The posterior drawer test is carried out while the patient is in a supine position and the knee is flexed to 90 degrees.
•The amount of translation of the tibia relative to the femur is observed.
The dial test is performed while the patient is in the supine or prone position and both knees are in 90 and 30 degrees of flexion. More than 10 degrees of external rotation indicates significant injury.
Common meniscal tears
Symptoms include
•Knee pain
•Pain with straightening the knee
•Swelling
•Locking
•Weakness

Today, I went into hospital for knee surgery. When I awoke, I was surprised to find a bandage wrapped around my throbbing head. The nurse explained that a student observer had fainted in the operating room and his head had smashed against mine on the way down.

I'd been suffering with bad knee pain for about 2 weeks, coming from the back of my knee & partly above & below my knee at the rear of it. I put it down to a strain on the hamstring, but it was really painful for a while in the mornings.i work manually & found one morning i was like a spectator on the job as I was working on hill like ground. However I was working on a few home projects over the following week & the pain in the area wasn't getting any better. I was getting into my car a few days later & i felt a crack like noise coming from my knee & instantly my knee gave in, I found it extremely hard to try & stand & bare weight, it took me about 15 minutes to get back into my home because of the extreme pain. I'm using crutches for the past 2 days, I realize I'll have to get to my doctor to get a proper diagnoses. This video has been a great help to understanding the injury I may have. Thank you for the information guys!

The treatment is usually ice, NSAIDS, decreased physical activity, crutches, bracing, therapy. If it is an isolated injury, surgery is usually not needed. However, when there is associated injuries with other ligaments, surgery may be needed to prevent future instability. Always consult your doctor before you make decisions for your medical care.

Thanks doctor for the lecture. Please, I underwent IM nailing surgery a week ago on my right femur. After the surgery, I discovered that I could neither bend my knee or raise the leg. More informative then all the other videos, however I don’t have an answer to what my issue is. Narrowed it down. Sounds like snapping/crunching when knee is flexed, only when my quads are also flexed. I can make the snapping/crunching within 3-5 degrees of full extention, 2 sounds right after the other. I can let my knee swing while standing and no issue, only when everything is flexed are there sounds.

Very informative video! I am a regular adventurer. Or at least, I used to be. Recently, I received a puncturing injury to my right knee due to an accident involving a misfired arrow. I'm able to walk on it and the pain is subsiding on some occasions with proper medication and less lollygagging, but my job has been affected as I am forced into guard duty while my cousin, whose knee is still perfectly intact, is out fighting dragons.

At time 5:29, can you please explain that we are viewing is the right knee, and at 5:42 that we are viewing TWO *right* knees seen side by side. Explanation could be in the form of marking the pictures, and also stating the facts in the audio track. The only reason I can decode the picture is that the kneecap is visible, hence frontal (=anterior) view, and that lateral means outside (to the side) and medial means inside (in the middle). Even better, show both right and left leg at the same time, instead of two right legs, and most of the opportunity for confusion goes away. Otherwise a great video, I'm going to finish watching now. Thanks!

Hello, thank you for this video. I had an accident where I cruckled on my right ankle then landed with full force on my left leg and felt an excruciating burn on the front of my left thigh and fell on the floor. I am waiting to see a specialist. However, I have snapped the quad off the patella before on my right leg 4 years ago (maybe I am prone to injury from OI type 1?) and I was wondering... If I can't do a leg raise and the CT scan said my tendon was still attached... What does this suggest?
