Knee replacement surgery is the most common joint replacement done in the world. In 2018, there were over 700,000 done!
Total knee arthroplasty, as it’s known in the healthcare field, is a treatment option for severe knee arthritis. It is usually done after other less invasive treatments have been done.
The after care for a knee joint replacement is HIGHLY important to make sure that the person undergoing the operation has a successful outcome. The usual way this is done is the patient begins physical therapy treatment in the hospital within 24 hours after the knee operation is completed.
Surgeons usually tell patients they’ll be up and walking within a day and back to “normal” within a few months. What surgeons don’t typically see however is that getting back to “normal” really takes 12 months.
It’s just that the surgeon’s involvement in the care of the patient ends a few months after the knee replacement surgery happens.
This video will overview what should normally be happening month to month in the year after having a knee replacement surgery.
Not everyone’s situation is exactly the same and people with other conditions like hip or back pain…or pain in the other knee, will usually change the expected recovery times.
Our 28 Day Knee Health & Wellness Boost Program can help you achieve more knee flexibility and strength. It is focused on fixing the common muscle imbalance that leads to knee arthritis and re-injury of a knee joint that has already been replaced.
It's a 100% online and on-demand program for improving knee health. It's packed with exercises, advice and coaching from Dr. David.

I had surgery four weeks ago and am doing very well. I came home the second day after surgery and walked right away. I iced my knee regularly, took oxycontin for three days, and was provided with several exercises to do daily. I have had PT three times a week with a plan to reduce the time to twice a week. I drove to my aftercare appointment with the surgeon the third week after surgery. I have 100% extension and 90% knee bend. I am 85 years old and a am very pleased with my progress.

Had my knee replacement on 14/04. To 18/04 I feel great.very sleepy. I refused to take oxycodone or OXY norm. It made me feel spaced out. I'm walking 3x 270 steps and climbing stairs. I do have discomfort but no real pain. My old arthritic pain is gone. I am very optimistic for the future. I am 76 years old.🎉😊

Great information, wish I had found it when you first released it! I'm 7 month post TKR and this was the BEST thing I've ever done. Yes I still have stiffness after sitting or standing but I'm able to walk with no pain for the first time in years and while I don't do my PT regularly, it definitely helps with stiffness. I couldn't believe that I went into surgery at 7:30am, was out of bed walking at 10:30am and was home by 12pm. The nerve block pump lasted for 2.5 days! At my last outpatient PT appointment at 4 months after stretching, I was at 0 degrees and 136 degrees. It took a lot of determination to power through the pain doing the daily exercises at home but it was worth it. I still have some occasional pain and my leg swells up if I sit too long. Currently walking 1.5 to 3 miles a day, losing weight and looking forward to getting back to regular life. Thanks again for the info!

Just had a TKR 3 weeks ago and can’t wait to get out and do some walking. The only problem I had was that the surgery team tightened the tourniquet too tight and bruised a muscle on my other leg. It hurt worst than the results of the operation. Walking with a cane after I got lectured at the physical therapy clinic. Long slow road to recovery but well worth it after having pain in my knee area after an work related injury 29 years ago, Keep up the good work and enjoy your new freedom. Be safe

I had bilateral TKR's 15 years ago. The 2 surgeries were 4 months apart, which meant I got to prehab the 2nd leg, while I was rehabbing the first leg. It made everything easier the second time around. For the first 2 weeks after discharge from the hospital, I went to a skilled nursing facility for 16 days for the first and 10 days for the second knee. This allowed me to have daily PT and it was covered by my Medicare. I am 15 years post op and i think the decision to have both knees done was one of the best decisions I made. I had tried everything else prior to the surgery and while the steroid injections gave me good relief, it was short lived. Definitely not a permanent result. I did not want to have both done together. It requires additional anesthesia and I was 70 years old. I am also a retired R.N. and knew patients who had done this and had a much longer and painful recovery. 4 months after my 2nd knee, my husband and I took a cruise of the British Isles and Normandy, and I did fine.

Thank you for this. My surgeon never informed me of any of this other than it could take 6-12 months to get back to normal. I’m at 5 months, exercise every day, am back at work, walk the dogs most days but the stiffness is driving me nuts. I feel it most when I walk down the stairs. My hope is going back for some PT with a different therapist will help. I’m looking forward to the day when I can say I’m glad I had the surgery.

Doing the exercises faithfully before a TKR is highly recommended. I did all my exercises before my TKR and the transition was a lot easier. I walked out of the hospital with a walker and haven’t used it since. I highly recommend using a cold therapy machine and a heating pad to help ease the pain. Keep a record or use a pill box for your meds since you take different medications at different times. A good recliner that tilts back almost flat is a lifesaver and will be your sleeping place for a couple of weeks. Trust me I been there.

@@anthonyrettaliata7311 Thanks. I also forgot to mention I use a small 2” x 6” paint roller with a roller to massage the muscles on the leg and thigh. I also used a vinyl boot to keep the incision area dry even thou they used a waterproof bandage on the incision. Elevate your toes higher than your eyes to circulate the blood in your legs area 2-3 times a day for 15-20 minutes helps also. I use a footstool and rest my calves on it. The first 4-5 weeks suck but in the long run it worth it. Your sleep patterns will be tough also. Good Luck and hope all works out well for you.

I feel I am getting quad stronger, but I don’t understand the swelling and stiffness that sets in. I hope in 15 weeks I’m feeling less stiffness and swelling. I’m asking because I have bone on bone in both knees, osteoarthritis and have lots of pain but I think some, particularly on the outer sides of knees and back of knees is coming from my back. My back doesn’t hurt though so it can be hard to diagnose. Gaba Pentin helps. I’m terrified to get TKR!!

I’m now 8 months in and although I’ve been following the guidance regarding exercise, icing etc, I’m unable to walk without extreme pain and swelling of my knee. Unfortunately physio was limited to only 3-4 sessions. I’ll be making another appointment with the consultant as I don’t believe this is part of the recovery process. Good luck with your operation. Make sure you build up your strength beforehand.

Wish I’d found this before my TKR three weeks ago. I live in the UK and I’ve not been given a breakdown of what to expect throughout the 12 months of recovery. Unfortunately I now know that I’ve grossly overdone where I should be in my recovery; I’m just hoping I can bring it back. Found the information on how the glutes impact knee pain very interesting and I’m going to seek out some exercises to strengthen this area. This might in fact be the reason my spinal surgery six years ago failed; hopefully it’ll help with the pain I still have in my back. Thank you!

I've had my knee operated on 6 times now, had a scope to see what was wrong with it after a motorcycle accident when I was 15, ACL repair, debridement, ACL replacement, another debridement, then finally a full knee replacement. After the replacement, the surgeon told me there was more damage than they thought, so they had to do more work than they planned on. Now, 7 months after the surgery, there is still a lot of pain, loss of feeling in my heel, and a feeling of instability. The surgeon said there is some movement in the joint, but nothing to worry about. I am still doing physio to try to get back to normal, but I find it hard as I feel pain and insecurity when trying to push the knee. Me and 2 physio therapists feel there is too much play in the knee, but the surgeon says it's fine. So, do I go with his word and just push through all the doubt and pain, or do I go with the 2 physio therapists and maybe get a second opinion? I had the knee replaced, hoping I could get back to a normal life, but not so. Now I am doing less than before because of the pain and unstable feeling. Any thoughts would be greatly appreciated.

I hope you are doing better now. It took me 6 months to convince my surgeon to do a CT scan on my knee replacement. He finally realizes the top part had not grown in and it had twisted around so far that the replacement knee cap was at the side of my leg. He operated again because he thought it may be infected. Once I was open the top replacement just fell out it was so loose, he had to dig the bottom one out. He put a spacer in impregnated with antibiotics and 1 month later that was all removed and a new replacement cemented in. I am now 3 months post the 3rd operation and my knee is swollen with fluid under the knee cap. All this has been going on for 11 months so if you still aren't happy get your doctor to check your knee.

I had a left TKR on Oct 24, 2023 and today (Nov 22nd) I am doing great. I actually stopped using any walkers or canes after the 2nd day. Still used the cane once or twice, but not after the 1st week. I think my prehab had a lot to do with it and also my legs have always been strong. I still have minor swelling, but going down. Last week I rode my bicycle 4 miles and it felt great, until the next day but not that bad. My ROM is excellent and getting better as the swelling goes down. My only pain meds after the first week are 2 ibuprofen and 2 Tylenol. There isn't really pain, just soreness and discomfort. I got the spinal block and recommend it. It took a few hours for feeling to return to my feet and I went home that day and have been doing exercises since. Great video and thanks.

Hey Bob! Thank you for sharing your experience regarding your total knee replacement. Glad to hear that you are recovering fairly quickly and are finding success when doing exercise. If you happen to be interested in a more structured approach that you can be confident in helping you return to the level of activity you desire, then you might be interested in our Failed Knee Recovery Program. Don't get caught up in the title, or the word failed, to clarify what we mean - is that we oftentimes see people have TKRs and still not have their knee issue resolved because the root problem was never addressed. We don't mean to say that the operation itself was a failure but more so the preventative measures or post-op care was not what it needed to be. You can learn more about the program here: "Failed Knee Replacement Program" This program works even for a revised knee replacement. You will learn: -Fast ways to reduce knee pain and swelling so you can quickly get to being able to tolerate sleeping and being on your feet more. -How to gain more motion in your knee joint (if you're missing some motion) without forcing yourself to do aggressive, painful stretches. -Thorough understanding of the root muscle imbalance so that you can apply the information to your specific life situation. -How to get the most out of your replaced knee joint while keeping it healthy for the long-term so that you never have to go back to another knee surgeon. www.epmanualphysicaltherapy.com/failed-knee-replacement-recovery/ We wish you the best on the road to recovery! 🙌

My PT appointments have been so painful that I’ve discontinued them. The technician pushing my knee to achieve total straight angle caused 10 pain level: I yelled out loud. Following this the quad sets too painful tears flowing at 6 repetitions.

6 weeks after surgery and have to go back for another surgery. Dr has to open up the original incision ( about 3 inches)and then re close it. The reason for this is because the original incision never healed correctly. I am on cumadin. 26:37 which causes slow healing

I had TKR in April, 2023 at HSS - considered the best orthopedic hospital in the US if not the world. Based on my experience, much of this info seems outdated. My incision was glued closed & I had no drains; when I removed the bandage 7 days post-op, it was thoroughly sealed. I’m sure internal tissues were a different story! But because I’m very active, I specifically requested the implant be glued in place to minimize the risk of them becoming dislodged. By the way: Studies have shown those flexion machines don’t provide better outcome, so they’ve been dispensed with. As this video advised, I was vertical within 24 hours - but there was effectively no weight bearing (much less walking). Everything was excruciating. Early PT was about moving my leg in ways to minimize pain, and how to leave the bed safely. Three bed exercises were recommended immediately and frequently: flexing toes forwards and back, pressing knee into mattress and clenching buttocks together. Extension was GREATLY emphasized over flexion - they want that leg at 0 degrees as soon as possible (within 2-3 weeks). My surgeon advised that flexion can be worked on over time, but to go gently with bending exercises: better not to inflame the joint just for the sake of rapid flexion - you’ll only lose those gains with the resulting inflammation anyway. She even said to tell my physical therapists to refrain from aggressive strategies (which advice one therapist took offense to). Extension is a different story - becauseg the longer the leg goes without full extension, the greater the chance it won’t reach it. Extension was monitored daily. They want you icing that leg completely straight with a towel under your ankle, toes pointed towards the ceiling. Your leg doesn’t want to do this, so I gave it no room to do otherwise: I wedged my leg between 2 towel rolls to keep it from turning, and made a “foot block” from a blanket to force the foot to stay vertical. As it became less sensitive, I wrapped both ankles together in a towel - and also slid both legs in a pillowcase to encourage extension. I reached zero degrees at 3 weeks. If I can do it you can too!!! Good luck!🌃

Did you have good pre surgery ROM? I had and managed 0 extension and 120 flexion from week 1 in both knees.
@jenniferlynn3537
@jenniferlynn3537 Ай бұрын
@@mariancoffey8271 Unfortunately no - and that surely was a major factor in how quickly my range-of-motion gains were achieved. It’s been a year since my surgery and my range of motion is considered excellent. It’s not quite what it was before my knee deteriorated, but I’m still working on that. I should just say that I was more flexible than most to start with - so my expectations may exceed what’s physically possible. In my experience, many of these advisories about the perils of not making rapid flexion improvements are a bit over-the-top. One year out and I’m STILL noticing improvements in ROM, strength and ability - though obviously on a much more modest scale. I believe that as long as someone is persistent, physically ability WILL come - and I wish videos emphasized this more. I worry that people will hit 6 weeks and think, “Well, I guess this is as good as it’s gonna get,” - then slack off because they don’t realize they can still recover more.

Full knee replacement in August 2023, September 2023 went back in to put a smaller spacer in and cleaned scar tissue out, October 2023 manipulation under anesthesia. Physical therapy initially 2x a week then 3x week and then 5x a week. Here we are now April 2023 I can drive a car, bend at 95’ and straightening to 9’. Can’t run because I’m off balance. When at PT can get knee flat and bend to 120. Once I leave about 30 minutes later knee “freezes” up, feels very tight above knee. No pain just major stiffness. Can bike. Using a Dynasplint for straightening. and bending. Graston at PT lots of stretching, biking. I do lots of passive stretching daily and exercises prescribed by PT. What can I do to get rid of this scar tissue? My surgeon says I can have a cleaning of the scar tissue removed for the third time. 😒 PT says if I have more surgery we are going to creat more scar tissue and may go backwards again. What are your suggestions? I ice and heat daily too. At PT now 2x a week.

My TKR surgery went so well my surgeon wanted me to do a video clip for him. Unfortunately after having an exceptional first month I got Patellar Tendonitis. Basically no treatment for it, just time. The doctor said 2-3 months. My situation was about 6 months before I felt relief. The pain from the tendonitis was so much worse than the surgery. I truly did not have much surgical pain. I live in an RV so I climbed stairs the day after surgery. It’s all in making sure you have proper instructions on what to do. The worst part for me was trying to find a comfortable sleeping position. It was almost 18 months before I could say I was happy I did it. I don’t think it would have been that long without the Tendinitis issue.

Hello again . Update 6month in and 97 % of my pain I had after knee replacement is gone .. 100% Of my initial bone on bone pain is gone. But at the 6 month mark, I'm still having some stiffness. And and some discomfort at the end of each day depending on how hard I worked. When I stopped doing rehab which I did for an extra 5 weeks on top of the initial 5 weeks I was at a degrees of 125 bendability. I don't know what I have now. Because I don't go to rehab anymore but it feels less I think.. Doctor keeps telling me that it's going to take up to a year but it will still get better. I will lose my stiffness . Would love to hear your opinion on this is sound accurathank you.

I had TKR Sept 2022. Everything seemed to be progressing well except about a month ago I started feeling a lot of clunking and clicking while bending my knee. Also I’ve tried to take the stairs more and that seems to have aggravated the area. I started glute strengthening about two weeks ago. Is the clunking normal and is new pain normal when doing stairs? Thank you for your help

Can you please tell people with fibromyalgia how much harder it is because of nerve pain ? Excellent video thank you, very reassuring, in the uk we seem to have less opportunity for more than 1/2 dozen physio sessions unless you go private which I am now contemplating and only one post Surgery Consultant medical review, so your channel is very helpful! I’m now 4 weeks post surgery, doing ok I think still feels like I’ve a heavy wooden leg attached 😳 I also feel like I have a thick tight band above and below my knee which is very painful when I’m trying to do my knee bend it’s almost as if my wound layers have been stitched too tight! Has anyone else experienced this? 🥰🏴󠁧󠁢󠁳󠁣󠁴󠁿🇬🇧 subscribed thank you

I am 3 weeks post bilateral replacements. I have 250 Flexion and 0 extension, but I have a similar sense of swelling, especially at night. It is settling but still disturbing sleep. I hope that it will settle for both of us.

@@mariancoffey8271 thank you Marion majority of that tight band feeling and pain thankfully settled by 4 weeks, now 6 months post op and doing great, full range of motion etc only minor swelling and discomfort by the end of the day and still feeling a little internal “clunk” going up and down stairs but absolutely no arthritis pain, hopefully that will go too😊 you are brave having both done my surgeon won’t do those so looking at a repeat show next year for the other knee, acupuncture is keeping the pain manageable at present it’s been a lifesaver 😊 wishing you all the best in your recovery and hopefully a good nights sleep soon! Easier to cope with all this when you’re not sleep deprived too.

Had my TKR 4 months ago. Still very painful when I first stand up. The longer I sit, the more it hurts when I stand up. Once I begin moving the pain goes away and I can walk normal. My surgeon wants me back in three months instead of a year to make sure the pain is better. Don't know if I should be worried yet.

Not upstairs to my shower for a month? I do have a downstairs bathroom but not a shower! First knee replacement scheduled for 7/31/24. I’m 67 with Fibromyalgia, RA, Hashimoto’s Thyroiditis etc. also. But I DO like to be clean, lol!

Have you ever treated a patella tendon repair and TKR at the same time? Went in for TKR and my patella tendon was torn and repaired also. I’m 4 months post op and only have a bend of 85 degrees. Doc is talking about scoping the knee to rid scar tissue. My doc and PT have never treated both at the same time. Any advice?
