Good to learn that u don't take medicines without prescription. But in India, self medication is VERY common. Many people blindly trust online info and take meds without a professional's advice or supervision. This is more common in Kerala. I see at least 2-3 patients every week who have "self medicated".

ഡോക്ടർ, ഞാൻ 2 മാസമായി ഷുഗറിന് മരുന്ന് കഴിക്കുന്നു. ആദ്യം hba1c7.3 ആയിരുന്നു. എന്നാൽ ഇപ്പോൾ അതു 6.5 ആയിട്ടുണ്ട്. ഇപ്പോൾ അവസാനമായി ടെസ്റ്റ് ചെയ്തപ്പോൾ FBS 100 PPBS 110 ആണ്.. പക്ഷെ അന്ന് മുതൽ ഇന്ന് വരെ എന്റെ യൂറിൻ അമിതമായി പോകുകയാണ്.. രാത്രിയിൽ 4 തവണ ഒക്കെ മൂത്രം ഒഴിക്കും. പകൽ സമയത്തു വളരെ കൂടുതലായിരിക്കും. ഞാൻ metformin ആണ് കഴിക്കുന്നത്‌. ഒരു ദിവസം 2 എണ്ണം കഴിക്കുന്നുണ്ട്. എന്നിട്ടും യൂറിൻ control ആകുന്നില്ല. ഞാൻ രണ്ടുമൂന്നു ഡോക്ടര്സിനെ കണ്ടു. യൂറിൻ ടെസ്റ്റ്, ബ്ലഡ്‌ ടെസ്റ്റ്, കിഡ്‌നി ഫങ്ക്ഷൻ ടെസ്റ്റ്, യൂറിൻ culture, എല്ലാം ചെയ്തു.. ഇത് കാരണം മാനസികമായി വിഷമത്തിലാണ്.. എന്നെ ഒന്ന് help ചെയ്യണം സർ.. എന്റെ സ്ഥലം kollam ജില്ലയിൽ ആണ്..

Sir എന്റെ അച്ഛന് ഷുഗർ 120 ആണ് .. Dr. Normal ആണെന്ന് paraju. metformin 2 ടാബ്ലെറ്റ് രാവിലെയും രാത്രിയും കഴിക്കുന്നുണ്ട്. Glicaside രാവിലെ 1 രാത്രി 2 ആണ് കഴിക്കാൻ പറഞ്ഞിട്ടുള്ളത്. ഈ തവണ check up കഴിഞ്ഞപ്പോൾ insuvin 50 കൂടി add ചെയ്തു. ഇത്രെയും tablets ആവശ്യമുണ്ടോ??

Yes... Januvia (Sitagliptin) 100 mg is a very useful med for Type 2 diabetes. It's safe, effective and well-tolerated by most patients. But yes, it's a bit expensive.

Sir, as you mentioned, in case of any infection it's better to avoid metformin tablets. Apart from surgery what kind of infections you mean? During such cases can you suggest some alternate method to control diabetes?

Thanks for your question. Metformin is best avoided in any stressful situations like major surgery, infections, etc. It is usually best to start insulin. By the way, surgery is NOT an infection.

(ഇവിടെ, സൗദിയിയിൽ കിട്ടുന്ന മരുന്നിന്റെ ലീഫ് ലെറ്റിലും അതെഴുതിയിട്ടുണ്ട്). പിന്നെ ഞാൻ ഇൻസുലിൻ തുടങ്ങി.എന്നാൽ ഷുഗർ നേരത്തേ പോലെ നിയന്ത്രണ വിധേയമായില്ല. പക്ഷേ ക്രിയാറ്റിനിൻ 1.2 എത്തി. വീണ്ടും കുറഞ്ഞ ഡോസിൽ മെറ്റ്ഫോർമിൻ ശ്രമിച്ചു. ഒറ്റ മാസം കൊണ്ട് കിയാറ്റിനിൻ 1.5 ൽ! പിന്നെ പരീക്ഷിച്ചിട്ടില്ല. ഡോക്ടർ ഇക്കാര്യം പ്രതിപാദിച്ചു കണ്ടില്ല.

To put in simple language, Sustained release (SR) formulations are meant to release a drug over a prolonged period, but not at a constant rate. Extended release (ER), on the other hand, release the drug over prolonged period at a constant rate.

Thanks for your query. I don't know precisely what surgery is planned for u. It's best to stop Metformin 48 hours before a surgery and replace it with Insulin. Generally, minor surgeries DON'T require metformin to be stopped. But, it's advisable to let ur treating doctor to decide.

No... As far as I know, Metformin by itself doesn't cause hair fall. But diabetes is a known cause of hairloss.

Thank you Dr.for the information..I used to take glimifix M-I (I-0-1) for the past two years. My fbs is between 90-IIO. Same time 1 used to take lipikind, Clopilet A150, Revelol XL for past 10 years following an angioplasty. Do you expect any interference among these medicines? Secondly another Dr. in u-tube advised to stop Metformin immediatly as it causses deficiency of Vit-B12.1 expect your Kind reply.

Yes, Metformin causes Vit B12 deficiency in a small percentage of patients.

The normal pH of our body is slightly alkaline, in the range of 7.3 to 7.4. Lactic acidosis is a condition where in the pH drops to low levels (acidic) because of an abnormality in the metabolism of lactic acid. It is a medical emergency. Sorry, I can't comment on Glycomet during ur Hemorrhoids surgery bcos I don't have the complete facts with me. Thanks...

Using metformin many years, I really fear about lactic acidosis from health programs last year. Please include about symptoms of lactic acidosis in one chapter

I fully agree that acidosis is a fearsome problem with metformin. But it occurs only in a very small percentage of metformin users, especially alcoholics, in severe stress conditions like serious infection and major surgery, etc. Overall, it's a safe drug. I shall try to include acidosis in an episode. Repax!

Dr, my cousin is diabetic since his 27 years of age. He is taking insulin 30 ml and metfomn 1000 mg/day. though blood sugar is not normal. He is very much careful about the diet also. Can u suggest the reason
