A thought regarding Case 2b and DWI. Would the DWI image/ ADC maps have shown restricted diffusion in the post-op collection, what did you intend to write as your diagnosis? As post op collections regularly have old haemoglobin breakdown products, that just ''mess' up' the DWI signal or actually restrict diffusion. Would you still say in your report just abscess, or would you include a phrase like abscess or possible old haematoma, cannot rule out suprainfection, correlate with inflammatory lab markers etc. Thank you and rock on!

If the DWI was bright and the ADC was dark, that makes it much more likely to be an abscess. However, you are right that blood products mess up the evaluation of diffusion characteristics by significantly affecting the underlying T2 properties of the fluid. So if it's an old hematoma, it can be very hard to tell sometimes. In this case because the DWI is dark, it is very reassuring.

Sorry to hear you have a bad spine. Hope the myelogram goes well. There is a separate video about how myelograms are performed: kzbin.info/www/bejne/bqHdepVuZZmbgMk They are interpreted very similarly to how MRIs of the spine are interpreted.

Great lecture, being an Indonesian where TB is so prevalent, when I see the fourth case, my mind goes like " is it Pott's?" :D
