I know that all joy mutants are often humans, but Twisty has brown fur, deer like body, and also he looked like a wild animal, he doesn't have any human features at all. Other mutants have human shaped heads or some things that makes you say "that's a human", but Twisty was different

I think Twisty is actually human, but with his back weirdly twisted (what a suprise). And to me it looks like the antlers are the spine that has been pushed out through the throat and twirled itself.

If you don't rest at the bonfire before downtown he'll appear, if you defeat him you'll get 200xp and 5 mags, it won't count as a kill. Or pay, him 5/20 mags

Late reply, but I think it only triggers if you got no Mystery Jerky in your inventory. It doesn't matter if you sleep or not, because I slept and I still had him appear outside the cave asking me to give him 5 or 20 mags.
