Kira and Dio have the exact same problem, they're​ both cautious but they let their ego cloud their judgement. Dio is very intelligent but his abundance of power leads him to be cocky. Kira is smart and experienced at hiding yet his cravings force him to make moves and he gets too exited when things go his way and it gets to his head. Dio is much stronger than Kira but Kira uses his tools more effectively to make up for that.

I think the ambulance thing is a problem of the anime, since words take real time and they didn't show him getting ran over immediately, it makes the driver look incompetent. What I think it's supposed to have happened is that since Jotaro punched him away in stopped time, the driver couldn't react to a person suddenly being in the way and thus ran him over, at least I think that's the idea, but since in anime they talk a whole lot that moment kind of got the shaft here.
