From what I can remember, it comes from the term, "jumping on the grenade". When a group of guys go out with a group of girls, one of them will pretend to flirt with, "the ugly one", to keep her distracted so the other guys can get with her friends. He's taking one for the team or, "jumping on the grenade". In that way, calling someone a grenade is kind of like calling someone "The Duff", but meaner. The term "landmine" came after as an unimaginative way to distinguish "fat and ugly" vs "skinny and ugly". It's really horrible stuff.
