Thank you for the great video, as usual. I have a comment on the clinical significance of alveolar-arterial-CO2-gradient (at 03:30). It is clinically significant in cases of increased alveolar dead space, for example, pulmonary embolism or pulmonary hypoperfusion as in cases of hypovolemic shock. An early sign we see on the mechanical ventilator in ventilated patients is a continuous decrease of the EtCO2 (this is the partial pressure of CO2 measured at the end of mechanical tidal volume and it represents more or less the PACO2) whilst the PaCO2 increases or remains constant. This could be an early diagnostic clue and warrants further attention.

Interesting, is it also possible to predict an eminent pulmonary embolism by observing the A-o2 and a_02? I would assume that the Alveolar o2 would decrease if there is lack of perfusion or am I mistaken?

I have a Qs...why base of lung is overperfused and apex is underperfused ??

Because bloodflow is greatest at the base of the lungs( overperfused) and ventilation is greater than blood flow at the apex of the lungs, both are decreased but perfusion is more decreased
