I wanted to tell you a story about a patient who recently contacted me for advice.
This particular lady was in her 50s in a very prestigious and high powered post and completely fit and well. She was married, had a daughter in Australia and had a wonderful social life with hiking with her girlfriends. Around Xmas time in 2020, she developed a flu-like illness and after that she started noticing that she was getting breathless on very modest levels of exertion. So she contacted her GP but unfortunately couldn’t even get an appointment for 3 weeks. Eventually she saw her doctor who examined her and suggested that she may have asthma and gave her some inhalers. She was asked to go back in 4 weeks to see how she was feeling. Unfortunately she felt no better and actually felt much worse. So she went back in 4 weeks and this time saw another doctor who asked her to stop the inhalers and suggested that it could be her heart and organised a chest X-ray and blood tests. After a few days she was called back to the surgery and saw another doctor who told her that the chest X-ray and blood tests suggested that she may have heart failure and that she needed to see a cardiologist.
This poor woman was completely stunned when she heard this and even before she could collect her thoughts and ask the doctor any questions, she was shown the door with the advice that she would hear from the cardiologists soon. This poor woman went home completely shell-shocked with her life turned upside down in an instant. As she hadn’t even had a chance to ask any questions, she went to the internet and looked up heart failure and her heart sank when she read about all the horrible things that could happen with heart failure.
Finally she saw a cardiologist who confirmed that she did indeed have heart failure and needed to start some medications as soon as possible and he would arrange for a heart failure nurse to see her. This took another 4 weeks and the nurse (another new face) who came to see her, asked her how she was (and she was a little better), increased her medications and said she was going to review her again in 8 weeks.
In the meanwhile this poor woman continued to remain completely confused about why her medications were being increased. She started noticing that she was more tired and not sleeping well. She wasn’t sure whether this was her underlying condition or her medications or that she was developing anxiety. She went to see her GP who said that he was limited with what he could give her because he was worried about how they could affect her heart. So her anxiety progressed and it started affecting her relationship with her husband. She stopped going out with her friends because she was told that alcohol was bad for her. She stopped hiking because she had been told to avoid any undue exertion. As she started spiralling downwards, it started affecting her employment. She was desperate to see her daughter but because of her diagnosis, her travel insurance became unaffordable and she started comfort eating, piling on weight which knocked her confidence even more and made everything so much worse.
Eventually, she decided to go and to see a famous professor of cardiology privately. This chap who spent no more than 10 minutes with her, looked at her medications and told her that she was optimally managed and nothing else was needed and discharged her. It was after this, she contacted me to ask me my advice and in this video i will talk about what i advised her.
This lady’s heartbreaking story shows the problem with modern day medicine. This poor woman had seen a multitude of different nameless faces over the course of 18 months and was in a far worse position both physically and mentally than she was when she had started. SHe was on 7 or 8 new tablets everyday when she had never taken a tablet in her life before her life changed and unfortunately she did not even know what these medications were doing for her. Most disappointingly, despite all these encounters with healthcare professionals, the patient felt that she had been left to manage completely on her own,
This is far from optimal management. This is the kind of care that enslaves and enfeebles a patient and makes the patient worse rather than better. Today i wanted to chat to you about what i feel optimal care for heart failure should be and how care when properly administered can empower and liberate the patient rather than contribute to what happened with this poor woman. The principles that i am discussing here can be applied to all chronic disease.

WOW! You are the most amazing Healer I have ever heard speak with such loving kindness towards humanity. I have SVT abd PVC's, no heart disease as such, you have helped me with all the videos I have watched that you have provided. "Gratitude " and peaceful mindfulness are so important to well being. I can relate to most of this and could not stop myself from crying throughout and nodding my head in agreement. Lack of time with my Cardiologist to talk through issues have left me with years of anxiety about my heart. My last Cardiology appointment was a 10 minute telephone appointment, sitting in the hospital car park, because he had an emergency to deal with at the last moment. This left me feeling more anxious and uncared for. It is so nice to hear Dr Gupta speak in such an empathetic way. A little bit of reassurance goes a long way. Thank you for this video. 🙏

Dr, you made me cry. I went and still going through a lot of these anxiety problems . The consultants who dealt with my chronic heart deseas were heartless. They made me fell awaful and very scary to do anything. I had to leave very good job and stay home doing nothing but worrying about my life. They left me with no hope at all and I am very depressed and having nightmare every night. They accused me of being mental and left me without proper supervision . Would love to see you when I am back home.

I can’t begin to tell you Dr Gupta how this talk is impacting me. I am a duplicate of this fifty something patient just a little more senior. I biked I hiked, paddled a kayak and camp and travelled to Europe extensively and one day in June2022 my life came to abrupt halt. I am being positive and your talk tonight is helping me! I do feel I have a great team helping me so I’m hopeful. My reduced ejection fraction is at 20 and I do now understand that’s not good. In fact bad. Listening to your videos has given me more hope!! Keep up your good work doctor I appreciate it from the bottom of my left ventricle.

Had a similar experience. The doctors made me worse for a while. Anxiety. Depression. Lost my sense of adventure. Began diet and exercise. Stopped all meds except a small dose of Amlodipine. A little better now. I'm 68.

This is a great video. I felt like you were telling my own story. What resonated with me is your description of what optimal care should look like. It is so different from the care I actually receive, with 10 minute appointments and a different doctor every time, just the same as you described. Also very little explanation of my condition or options for managing it. It’s more like I’m given marching orders: do this and not that, take this medicine, etc. And as the poor woman experienced, I seem to get worse with every appointment, test, and medication. This new year I resolved not to see any doctors or take any but the most basic meds. I realize that might not be the best approach but I was at my wits end. After watching your video I have new ideas about how to cope. Thank you!

I'm 73 and never had a health checkup in my adult life I don't want to no what would be killing me. Surprisingly I'm still fine and healthy while everyone around me have either died or unhealthy. I figure 10,000 years from now, living a few extra years doesn't mean that much. I've been sick before that I thought I would be dying soon but I didn't die.

I am binge watching your videos, as I try to come to grips with my Heart Condition (on top of my Stage IV CRC diagnosis). You have also made me realize that I have truly fallen on my feet, when it comes to my Cardiologist. He listens to ME. When he tells me he wants to try another medication, and I tell him I want to try a healthier diet, he listened, giving me two months to improve my LDL number. He also spends at least 20 minutes with me, and I never feel like he is pushing me out of his office. So, I thank you, Dr. Gupta. I feel like I have two Cardiologists, and I love them both.

I know what you mean. I have Stage IV Cancer and just topped that with a heart attack. While I am OK during the day, keeping busy, my nights are fraught with fear. Fear that the cancer will progress. Fear that I will have another heart attack. When Dr. Gupta said "Fear won't stop you dying, but it will stop you living" I'm like yes, I need to embrace life and let the cards fall as they may. I hope you can live without fear, we all deserve that.

I look at my Cardiologist and try to find Dr. Gupta but I haven't yet. I'm a patient, a canned routine 10 minutes, a duty fulfilled for the day. The Healthcare System can't afford Dr. Guptas. God bless you, Sanjay, for trying to make it better.

Dr Gupta thank you for your video. I am a sufferer of heart failure for over 30 years and it is only 5 years ago when my brother has been diagnosed that I finally began to talk about it. I have been a prisoner to it because no one else knew exactly what I was, and still do, go through. I was 22 and with a new baby and my life was turned upside down. My cardiologist was very sympathetic but other that once a year check ups and ‘mot’s’, and pills were all I had. So I internalised it for so long, I know it has damaged me mentally. So everything you said there I feel is very much needed. Thank you again for saying what we feel inside

About 4 years ago in my local NJ paper was an article about a 102 year old daredevil named Kenny Meyer who became the world's oldest skydiver. He would start each day with 2 eggs sunny side up. He was very health conscious and fit and a wonderful man to all that knew him.

In three weeks I have my first appointment with a cardiologist. I am mentally preparing for a fight over medication and my perceived quality of life. . If my appointment was with you I would be preparing to show you a photo of my dog Gypsy and my cat Tom, because they are important to me as a person. Thanks Dr for your videos. I wish you practiced in the USA. I totally agree with what you are saying. More physicians should understand that they are treating individuals and not just another faceless patient.

Thank you Dr. Gupta! For your compassion and respect to each individuals circumstance. My story is that of a highly conditioned marathon runner and XC-skier. I competed at a high regional level with a marathon PR of 2:26 and at age 29 covered 50 miles on foot in 5hours 44 minutes. The wheels fell off that wagon abruptly at age 64YO. I went from completing a 42 K ski race to Cardiologist performing angioplasty and placing 2 stents, all within a week. 3 days after that I endured and survived a heart attack. Never in my life had I felt such intense pain. Im 70YO now and am happy to raise a robust veggie garden and help tend 3 amazing grandchildren. My heart has been in paroximal AFIB for nearly 30 months now. Calcium channel blocker and anti coagulant have helped greatly, but in all likely hood an ablation is on the near horizon. So ,there you have it. Eat healthy, exercise regularly, and still encounter challenges.🤷‍♂️ But for the grace of God and Dr. Gupta, there go I.🕊

Hi Doug-I agree Dr. Gupta is one the best patient advocates. I am 72 and have been active for almost 40 years running and playing aerobic sports like handball. I developed atrial flutter Sept. 2021 and went from active person to getting short of breath taking a shower, walking upstairs, etc. Cardiologist did workup and put me on beta blocker and anticoagulant. Still felt poorly and dosage of beta blocker was pushed higher and higher until I was clinically in congestive heart failure. I tapered my medication slowly downward being retired GP until was out of failure. After almost a year I asked for DC Cardioversion and have been on sinus rhythm since August. Some studies have shown long term exercise sets us up for atrial fibrillation while walking/modest bike riding does not. Here is one article but there are others: pubmed.ncbi.nlm.nih.gov/29891251/ Longstanding atrial fibrillation can to tachycardia induced cardiomyopathy but can reverse with lower resting heart rate. Ablation used to be used after medication failure to address atrial fibrillation but the medications all have side effects and effectiveness diminishes over time. Ablation is starting to be used earlier in course of the disease. Sorry for the long post but I identified with your situation.

Wow I would do anything to be able to see you about my heart failure. I’m 45 now and was diagnosed aug 29th 2021 with end stage heart failure. In August I had a EF of 8 to 10 % and working with a transplant team at Oshner a hospital in New Orleans Louisiana. Today I have a EF of 35 to 40% but I’m dealing with this mentally completely alone. My husband of 10 years left me and turned to drugs and I find out as I was in the ICU first finding this horrible news he was also cheating on me which made all this more horrifying. Now I don’t even know what to do to save my own life when I’m not sure I wanna even live one anymore. It’s all so so much to handle 😢😞

Going from 10 to 40 percent is pretty amazing, keep going. I wish we all had good doctors who were inspirational. I wish we could all share our stories of what helped or hindered our health success.

Thank you Dr Gupta. My husband is being taken care of a HFU team and your video give me better understanding of what to do once he get out of the hospital. Also, I need to take care of myself as Mayo clinic forget about me(even after going to the emergency after 4 days ago). I called for an appointment but just told me, cardiologist is not guaranteed. How rude. So far blood tests are good but I know there is something wrong with me. I will find a cardiologist who truly is concern about my heart issue. I am at times out of breath and exhausted after a morning walk.

I was told recently that I had heart failure. I just thought that meant I'd be dead in a few years and decided to prepare for that. After hearing what you said as well as reading the comments I'm not so sure anymore. Thank you Doctor and contributors! 🤩

Man. This amazing doc is so right that this is scary a bit....because this is true. Medical conditions can collapse your daily routines and your life as a whole. You become consumed. I know that I'm consumed by a damn high blood pressure reading on the fucking blood pressure monitor let alone heart failure or something worse like cancer. Crazy. You just to take bad news like this and somehow embrace it and be proactive and try to relax and get as healthy as possible. Nothing more anyone can really do.

I’m quite lucky to have a cardiologist that works well with me but I consider you my other doctor:) Your videos in part inspired me to lose over 100 lbs and put my AFib in remission for nearly two years.

I agree. Continuity is key. Also information. Before mental and physical health spiral's. I have a chronic long term illness. My husband has cardio issues. I agree with your Optimal care with patient led care. If only all Drs felt like you do Dr Gupta...we used to see one GP. Our surgery no longer allows that. Which i told them i thought this was wrong...building that relationship with your Dr makes a huge difference to how a patient copes with chronic illness. You really are in a league of your own amoungst Consultants...so caring and thoughtfull...Do you treat patients privately through vidio consultaion Dr Gupta.? The whole persons mind and body are all connected in treatment.... the body is a whole. Each person an individual. You explain so well for the normal non medicaly trained patient.

No one explained anything to her. There are different degrees of heart failure with various presenting problems. Thank you for your time and explanations. O2 needs measurement. Heart and lungs are so connected especially in heart failure.

I feel this acutely. I have been having a range of symptoms for the last 3-4 months. Palpitations, constant fatigue, bradycardia, and malaise. I have barely seen any doctors in these last months. All the while I am scared and my anxiety has spiked as I am left feeling awful and have no idea what’s wrong. I do have POTS but it’s been under control for the last 10 years. My doctor just wants to put it all down to anxiety even though me and my therapist disagree with this. I feel so lost :’(

My doctor told me my irregular heart beat was anxiety but I know it's not. They can't even pick anything up on the monitors and I went to 2 cardiologists. I pray you get down to the bottom of whatever is going on with you 🙏

What are you waiting for? Half the time I tell my doctors the tests I want done. And I spend several hours researching each condition on KZbin and Google then make a summary sheet to take to my doctors. I list names of medications the ones I might need as well as the ones I'm taking, lab and imaging tests, etc. I always read up on medications from dosages to contraindications and interactions as well as toxicity levels listed on the FDA drug page. I also ask questions of my pharmacist to see if it matches what my doctor says. Don't be lazy, don't play the helpless victim, spend time and do your research. Doctors are sometimes really really stupid and lazy as f***, some are having a crisis in their personal life like a death or divorce and that filters into how they treat you. They rely on pharmacy reps selling a medication instead of doing their own research. If a doctor is condescending, I leave them in the dust and move on, same if I think a doctor is unhealthy like obese and hypertensive. I once had a primary care doc who said she treated her own hives with steroids when she needed too and that her brothers was an allergist. I thought she was stupid for using toxic meds that only suppressed her symptoms without finding out what the underlying cause was, never went to her again. These days you can get a lot of opinions from telehealth doctors too. If you want good answers, you need to ask the right questions and if you don't do the research you can't formulate an intelligent question that rules out what it might not be. And if you need a second or third opinion you may have to pay out of pocket. Get self-monitoring home devices like Kardiamobile, a recordable pulse oximeter, learn to read your labs. I recently had a nice chat while undergoing an Echocardiogram and the tech said some doctors sometimes end up being influenced by a techs report which could be wrong and it causes them to give the wrong diagnosis. I nearly ended up with an unnecessary open heart surgery until I got a 2nd and 3rd opinion. Talk to your insurance about second opinions. Talk to nurses and techs about which doctors they respect. Even talk to paramedics. Then make up your own mind. Too many naive patients rate a good doctor just by their bedside manner, that's often not enough and you won't know unless you walk in with a stash off detailed questions.

I had COVID last month . I have a left brundle heart block since 1993. I’ve had no energy for years. My cardiologist recommended a pacemaker to try help my fatigue. It didn’t help I also had a heart attack in 2008. I have been in Carvedidilol and Spirolactone for a years. Last year I also had COVID and my blood pressure was really high. My cardiologist did numerous tests and increased both meds to get it under control. This time my doctor took me off my heart meds and put me on Isosorbide Dun 5 mg twice a day. I have a headache most of the time. Also difficult to breath. He sent me for a chest x-ray and I go tomorrow for an echocardiogram. I have asthma but not real bad. My doctor seemed perplexed about my condition. Every time I stand my blood pressure and pulse rate go up. I’ve been in bed now for 6 weeks. I feel like the lady you told about on the video . I also have chronic back pain and had two back fusions. I have depression, anxiety, panic attacks & PTSD. I can’t exercise and have gained weight. Don’t know what to do.

You can find videos from doctors about post covid POTS. When ever I hear of someone talk about asthma l think about low vitamin d levels, I would also think of this when hearing of back problems, along with other nutritional deficiencies.

Dr Sanjay you are doing a gerat job and truely justifying your profession, I am in India, my father is having Heart Failure and Afib, recently He has been through CABG + MVR ( replacement), But We are still not sure the treatment being given by the doctor, The doctor can not convince us and keep changing medicines, Its like they just want to take money from us, My father sometime doing good, and sometime his water retention increases etc

I was diagnosed with chronic heart failure 2 months ago. I can relate to all of this. However, I am under the care of the heart failure team at my hospital and part of a family. I also started a WhatsApp group with local people who have this condition. I feel fine.

I would like to know too. I was diagnosed with sarcoidosis last year and am now taking 7 different medications. My blood pressure shot up so I have ramipril. I feel worse than I did before starting these medications 9 months ago. Now my pulse rate has rocketed and I am short of breath. I have no idea if it’s caused by the condition, the medication, or some new complaint. My world has shrunk to housebound with a 200 yard walk with a stick if it’s a good day. Just listening to these videos isa real comfort and encouragement. Thank you so much Mr Gupta

I am in constant Afib. Have been for a few years. My cardiologist thinks it would be very difficult to stop it now. But, I have adapted to it and it does not bother nor hinder me at all. So, I am chronic, but feel fine.

My average is 149/87.3 on coversyl 10 mgs none smoker social drinker , good diet and exercise around 90 kilos . I also have a walking machine and can walk reasonably fast for 40 not out of breath. Would you say this is ok? Before meds was 170/100, it worries me, because it can still go 160/100 at times, but listening to you makes me feel better, I was a nurse and know how they push medication, would be grateful if you could advise. I’m 59 years old. Thank you for all you do on here, I must admit I was about to book a consultation with a cardiologist.

You are the best Doctor I was facing difficulties in breathing and went to see my private doctor and he suggested I do a ECG I did and it read i had early and benign reploralisaton and he sent me to a cardiologist who put me on beta blockers then later diagnosed after I had a heart monitor for 1day that I had supraventricular tachycardia. Am so scared cuz my life seems like it will soon end am 30years I really need help

Dr Sanjay I agree with you . Here in India a specialist doctor shares his personal mobile No . One can contact him on sundays or by what’s app Lot of humility and humane approach visa vis foreign doctors . I experienced both . Indian doctors are best
