I am now 10 days post op and four nights into taping my nose and I honestly see a tremendous change in my nose and how I wanted it to look like! thank you so so much!! I was really worried that I might look like a dinosaur at first but now I’m slowly seeing my nose change into my dream nose ❤️

Thank you for this great video. I had an open rhino plasty and loved my results until after going to the gym my nose swelled up significantly. That happend 5 months ago but my nose hasnt really gone back down to how it looked. my doctor said i should massage which I am doing but like you said in your video I still wake up with a swollen nose. I am black and have thick skin. would taping still work for me at this stage. I had my surgery 7 months ago.

Hi. Swelling can last up to a year. If you have thick oily skin, you may benefit from topical Retin-A cream to reduce oil production. This can thin out the skin a bit. Another option is the use of low dose Accutane. Occasion you might benefit from steroid injections. Speak with your surgeon about these options. Good luck.

My surgeon told me to wear tape for 6 months 24/7. I can't wear it 24/7 because I have to go to work and I don't want to look like a freak but yeah it just shows how important it is to tape all the time

Thank you!! I love KZbin. My Dr hasn’t recommended any of this for swelling. I am 9 weeks post and still seeing a lot of tip swelling. I have green doing the massages and will now include the taping. I will also inquire about the splint when I return for my 3 month f/u He is out of town so it makes f/u a tad more complicated being he hasn’t seen me since a week post.

My patients start as soon as the splint is removed. They perform the exercises for 2 months until the nasal bones have solidified and 80% of the global swelling is gone.

My patients go back to the gym for cardio at 2 weeks postop and then start weight lifting at 4 weeks after surgery. Exercise will increase swelling so I recommend icing after exercise. Good luck!

Can cleaning the inside and outside of your nose with an earbud or your finger nail cause damage like 1 week post op (just after all packing, splints and cast is removed)?

It really depends on how aggressive you are. Gently cleaning with moist Qtips is the safest way to go in my opinion. Too much pressure placed on the nose this early might shift the soft nasal bones into the wrong position. That's why my patients aren't allowed to blow their noses for 1 month after surgery. They can wash with bottled saline or use a Neti pot for cleaning gently.

Hi I have a question, I was very impressed with my result after the splint was removed at day 7, nose was nice and straight with a refined tip, but now despite strictly following after care instructions, at 3 weeks I can see the middle of my nose start to grow a slight hump like the one I used to have pre rhinoplasty, and im worried as Im not sure if this is the actual bone thats swelling or cartilage, id appreciate if you could respond when you have a chance thanks!

If your surgeon is ok with it, I would recommend secure taping of this area at night to shift it into a more straight and flat position. During the day, the tongue depressors can be used to iron out/squeeze out swelling in the cartilage and skin.

Dr Shah, I wish that I could come and see you! I am in Kent, England. I am over 3 years post-op after a septorhinoplasty procedure. Swelling is almost the worst it has been! I have not been advised by my surgeon to do any massage techniques, only taping (which I only started doing 18 months post-op, I wasn't advised to do so sooner). I deeply regret my decision to have the procedure done. :(

Hi Maria. I think the best way to reduce scar burden is to control swelling with the techniques I've described. Also control the oily nature of your skin with retinols.

I know this is gross but you might want to take a pair of tweezers and pull the boogers out. My surgeon did this for me since I wasn’t blowing my nose (you’re not supposed to) and they were all stuck up there making it hard to breath

Cheryl Ferry yes you should be concerned. Ask your surgeon for another metal splint to wear first and then wear your mask. Or tape your nose down during the day and then wear your mask.

I have very thick, oily skin and my surgeon reduced my nose size by a large margin. I'm at about the 4 month mark and it's bumming me out because every other week it swells up. It will start to go way down, and when it goes down it looks beautiful. Straight and small. Then I'll make the mistake of taking a couple shots of alcohol and hitting the gym, thinking maybe I'm past the point where it will keep swelling. Then, just like clockwork the supratip and tip swells up the next day and stays that way for a week. Then it goes down again. I understand at this point I'm pretty much just waiting for the skin to shrink and fit my nose. It's just driving me crazy because I feel like I can't live my life while I'm waiting for the nose to settle. I'm always worried if I touch it the wrong way or raise my blood pressure that it will swell up and never come down. Am I just being paranoid? Will there be a time that this swelling will stop and the nose will shrink for good?

Hi. Be patient. What you are experiencing is very common in oily noses. A little trick I use for my patients with oily skin is the application of OTC hydrogen peroxide on the T-zone of the face at night to kill the bacteria in the pores and reduce the oil output. It works wonders. You should go ahead and live your life without worry. On the days that you know you've eaten things or done things that will swell your nose, perform the massage and tape overnight to hold off the swelling so you don't find yourself fretting for a week. As long as your skin doesn't get irritated from the paper tape, you should do fine. This combination of actions should give you incredible peace of mind when followed intelligently. Good luck.

Yasmine Tete take a break from the tape to let your skin heal. Once the skin is healed you can try medical paper tape. But if you have a tape allergy, then skip the taping portion and just use the massage.

I had a deviated septum from a jaw surgery that I had, and had a septorhinoplasty done. My dorsal hump was removed, and septum was corrected. I kept my splint, and used it whenever I had my glasses on. However, my doctor never explained any of the things that you are explaining now. I wish he did. The tip if my nose still looks bulbous. The hump was fixed, but now if you look at my nose, it either looks like it has a very slight hump, or as you get to the tip, looks like it has collapsed a little. 🥺

How accurate are the surgery table photos taken right after the procedure is done under anesthesia? Can I expect those results to be similar to the final result I will see in one year? I had a tip plasty with a rib graft inserted into my tip and I really love my after results that I saw in the operating room table pictures, however right now I have a lot of swelling due to my thick skin and I was just wondering if there was hope to have a similar result as to the surgery table pictures. You will need to be vigilant and stay on top of your swelling reduction strategies to get the result you saw on table that existed before swelling and scarring set in.

I just got my cast removed. I’m 9 days post op! And I feel hideous. My nose is like a potato now. :( also when he removed my cast I still had a slight bump on the bridge :/ should I be worried. He explained my skin is very thin and there was a lot of swelling still. Also he showed me my after picture while I was under and there was no bump. So I hope it goes down 🤞🏼

You are very early after surgery. The slight hump is swelling under old hump skin. That skin needs to shrink wrap. If your surgeon is ok with, use my bridge taping regimen I show in the video to manage that skin and get it to lay down nicely. In ethnic noses, swelling can be quite excessive so be focused. Increase your protein intake and cut out salty foods/alcohol. Ice down after exercise or sex. These little hacks will help control the shape of the nose more effectively than just waiting for the skin to de-swell. The bridge will de-swell in 2-3 months and the tip in one year, so be patient.

Just had a skinny nose with a hump. He gave me a slight slope and made my tip slightly smaller to match my new nose! I’m seeing improvement it’s just the bump that is slightly there! It depends on how the ear cartilage was used. If your surgeon gives you permission to tape and massage then please do it. It can be very effective in the majority of postop situations.

if someone doesn’t live near you but wants a rhinoplasty from you , how soon after a consultation could you get the surgery?

Hi. Great question! I have patients who visit me from out of state regularly. We usually start with an online consultation using Skype or FaceTime. Once I know what you are trying to improve with rhinoplasty, we come up with a plan together, and my staff send you a price quote. If you decide to go forward with surgery, we do a preop appointment online. Surgery is paid in full at preop. We then have you come into town the day before surgery to sign all paperwork, take photos, and perform simulations. The next day is surgery and I have you stay at least over the weekend. Ideally I would prefer you to stay one week so I can remove stitches. But if you have to get home to recover, we try to find a Surgeon comfortable enough to remove stitches, etc. near your home.

I just had rhinoplasty literally a week ago and I was so excited at the cast removal but I started crying cause I feel like my nose looks exactly like it did before. It's not as crooked as it was maybe, but I can still see a little bump on it, and it's DEFINITELY not straight like I wanted it to be. The nurse said this is just due to swelling, and once the swelling goes down the bridge will be straight. I don't know what to believe. I'm confused, depressed, and mad cause I feel like I paid a lot of money for nothing. Has any of you gone through something similar? Will my nose look better once the swelling is gone?

Buy something called a Rhinoshield to protect your nasal bones from getting pinched by your reading glasses. You can wear the tape during the day for no more than 2 months after surgery.

Your nose will swell and deswell cyclically over the next 10 months before everything settles down. If you have thick skin some options might include: 1. topical Retin-A cream on the nasal skin or 2. low dose oral Accutane. It can help thin out the skin and reduce the oilyness. Those with thick, oily skin will have swelling the longest.

@@jammiejo185 you are welcome. The edges of the nasal fractures may always be palpable. You just want them to not be visible. It takes some getting used to. If you can't ignore the feeling of the edge, it can be adjusted in a year under local anesthesia.

I’m nearly 4 months in an I’m so swollen still , I get headaches all the time an inside my nose is all swollen an red , I’ve been to see the surgeon and he said it looks fine but now I have these small bumps along the middle of my nose an it sticks out an also a small bump on the side of my nostril that looks awful , he said it will go in a few months an that it’s either scar tissue or pockets of air because of the swelling and my nose is still very stiff , I’m not happy with my nose at all an it’s really worrying me that I’m stuck like this esp with the bumps to i feel like he’s not telling me the whole truth an says things to get rid of me , I have a hump on my nose an at first it had completely gone but not with the bump I’ve now got it looks like I’m getting it back again 😫

@@kellylovejoy2169 you really shouldn't be all that swollen at 4 months. The tip area definitely could be swollen, but the bridge shouldn't be. Full rhinoplasty recovery takes one year. Hi doctor, just about 4 full months post op, closed Septo/Rhino with bridge narrowing and tip refinement. I had a wide bridge with a bulbous tip. I wasn’t told whether i had thick or thin skin so I’m not sure. I also wasn’t told by my doctor either way, whether to tape or not, and I didn’t know at the time to ask that question. I still get “achey”’along the sides of my bridge, maybe due to what I ate or drank that day, I’m not sure. I also still have a great deal of swelling in the tip, it’s still very round w slightly more swelling on the left side of the tip so it appears crooked, even more so when I smile. From underneath it looks boxy and not refined at all. I wish I had taken photos immediately after cast removal bc I remember being happy w what I saw that day, not realizing how much was going to swell and forget what it looked like! I know I’m still early in the tip healing process but I feel like I’m not seeing much change in the tip now. I only had a 1 week follow up for splint removal and then a 1 month follow up and I was discharged 🤷🏻♀️. no other appointments unless there’s a problem. I thought that was odd, but maybe varies by doctor. My question is, would taping and massage at this point still help? Like I said he never said NOT to do those things. Thank you so much in advance. Please help bc I pray that the results I have right now are not the final! And I will be making an appointment soon to see him.

It won’t hurt to try. Tip swelling leaves by 12 months postop. Also ask your surgeon for a Medrol dose pack of oral steroids to help accelerate the reduction in tip swelling.

It's hard to know. Just wait until your surgeon says it's ok. Or ask about nighttime taping at least. At four weeks your nose is solid enough to tolerate that. That being said, I don't know what you had done, so I'd be conservative.

How much work can a doctor do using local anesthesia?!

Depending on the patient, I perform the following procedures in office under local anesthesia: facelifts, browlifts, tip rhinoplasty, ear pinning, cheek implants, chin implants, lip implants, buccal fat pad excision, facial fat transfer, small areas of liposuction (face and body), breast lifts, mini tummy tucks, and reverse tummy tucks. All patients use Valium and Pronox as well to bring comfort during the numbing injection process. It is incredibly well-tolerated and most patients tell me that the average pain score for the procedure is between 2-4 total for the whole procedure.

Hello octor ! Many thanks for this video. I m 2months and a half post op and feel lumps under my bridge right where my septum was corrected. Moreover I feel like bones on both sides of my nose where the hump was removed. Therefore my 3/4 profile looks irregular. Do you think that might be swelling for both situations or it looks like it may need a revision including osteomtomy to solve the second problem ? Thanks for your advice if you get this message. You might be describing an open roof deformity. But thin skinned patients can feel lots of detail as well. Is your skin thin?

I just had fat grafting on my upper nose and the tip 4 weeks ago, however my tip looks huge and bulbous, the doctor told me it’s swelling and that very little fat was placed. Will this massage help or will I need surgery to correct this. Can’t wait to start this 🙂

Hi :) That is actually quite common. It is due to swelling. Once the swelling subsides the tip drops into place. It can also be due to the amount of tip cartilage removed. The more that is removed, the more the tip naturally rotates to fill in the deadspace.

Doctor, I have a septorhinoplasty and my just removed my cast two days ago. I was told not to touch it at all for the entire year. But the swelling in my right side of the nose is growing (it was the side that my nose tip shifted to from my injury that led to the surgery). It was an open-surgery. Do you recommend I do the massage and tape or should I not touch my nose?

I would clear it with your surgeon, but my patients start with taping and massage on post op day 7. Too much pressure can damage the septum.

There are only a few reasons why you might have a bump after your rhinoplasty: 1. not enough hump was removed, 2. you are developing a periosteal reaction in the bone and it is swelling, 3. there is a small particle of bone/cartilage that got left behind, 4. your surgeon placed a cartilage graft and it is now showing, and/or 5. you are just seeing swelling of the skin as a bump. Depending on where you are in your recovery since your nose surgery each cause has a specific remedy. I would recommend that you speak with your surgeon. Often touch-up procedures are quite simple. Hey, I had my rhinoplasty 2 weeks ago. I had a deviated septum and a bump in my nose. Around 1 week ago my surgeon removed my splint and revealed my nose. It was very swollen, as expected! Everyday I’m seeing new things on my nose, the latest being the fact that my nose is still a little crooked! I’m not sure if it’s going back to how it was before or if it could this be due to swelling coming down on just one side? Could you please advise, can swelling make you see things? thank you in advance sir.

Getting a purely straight nose on a normally asymmetric face is pretty much impossible. But swelling this early into your recovery will hide how much improvement you actually have.

RNS it is not unusual to have an upturned nose due just to swelling. When the swelling goes down, the tip drops. At two weeks postop, you have way too much swelling. You should ask your surgeon about a steroid taper to speed the deswelling process after one month.

RNS you can wash your nose out with a product called Alkalol. It is a nasal/sinus rinse that you can purchase over the counter at most pharmacies.

Hi! I just took of my cast today, and I have 2 bumps on my nose that can be felt under the skin, but that are not visible to the eye. They feel hard, but at the same time a slight bit squishy. Is this normal, and will it dissappear when the swelling goes down? I love your videoes btw!

If they feel squishy, they are probably temporary swelling or suture knots under the skin. They will probably flatten nicely in the next 8 weeks. If they get harder and stay bumpy, they may be related to excess cartilage or bone particles under the skin, or insufficiently removed bridge bone/cartilage. In this case, revision might be necessary. Only time will tell. Good luck :)

Hi. In general, if someone has a hump only rhinoplasty and that hump was the reason their tip was not droopy in the first place, then removing the hump removes the lift on the tip and the tip drops. Preoperatively, this is known as a “tension tip”. If it doesn’t improve, it is relatively easy to rotate the tip back up and shorten the nose in the office under local anesthesia.

Hello Doctor, I had a rhinoplasty 7 months ago and I am still swollen and been having frequent sinus infection and congestion. my nose has been swollen ever since the surgery and it's been 7 months. Is this normal to have frequent sinus infections and congestion as well as being swollen after 7 months? I been using nasal sprays. But every time I catch a cold, it is back to square one with the swelling and congested feeling.

I am 4 days post op from cast removal. My doctor told me not to tape my nose. I have thick skin & my nose is very super swollen which I expected. My doctor told me the tapping isn’t necessary since it can pull and tug onto the skin so letting it naturally go down is better for me.

Surgeons have different opinions on taping and massage. Just follow your surgeons rules. That being said, thick skinned patients do the best with taping. You will need steroids most likely to reduce your swelling and that carries a risk of complications.

Hello Dr. Manish Shah, I had a revision Rhinoplasty done 2 weeks ago and my recent doctor had to take out a what we discovered was a too large of an implant on the bridge of my nose extending to the tip. It was so long that it was very noticeable and if I hadn’t taken it out it would have pierced out of my nose. My doctor was able to perform the removal and use my own cartilage to repair the tip, etc. I’m very worried though now because the tip of my nose is a light colored lilac color. I’m hoping it’s bruising from inside/outside and not damage to my blood flow for my nose. Is it normal to have discoloration just at the tip after 2 weeks? Thank you in advance if you’re able to answer.

Arnica gel may help. Taping can also show you the bridge that you will develop by one year. If it looks wide it may be swelling, but it may also be how the surgeon left it. The taping can help you see the difference.

Armenti R many surgeons don’t require their patients to do this. It is my belief that taping and gentle massage helps guide the healing. If your nose looks perfect to you then I’d recommend staying the course. In other words, if it’s not broke don’t fix it.

Hello, I’m in my 5th month after the operation. I’m still doing massages quite regularly because I have a very thick skin, it is not very very swollen right now, it gets better day by day. BUT after I wear surgeon mask due to covid19, the tip of my nose got swollen a lot. The tip immediately gets wider and swollen even though I wear the mask only for max 30mins in a day. It again gets back to normal after I do massages. But I am confused, is it ok to wear surgeon mask? Or should I wear those transparent face covers instead of surgeon masks?

It is ok and safe to wear masks. Because your nose gets warmer under the mask, it swells up. Your tip won’t settle down to its final smaller shape for 7 more months. After you take your mask off, try gently icing the nose first. This will get it to shrink faster. Also your thick skin can be managed with topical Retin-A to resurface the skin and thin it out a bit more.

Hi Doctor. I am 7 days post op surgery. I had a septorhinoplasty done to fix my deviated septum and reduce the dorsal hump. He also did a very slight tip lift and my doctor said I needed the columella shaved in order to have a better profile. I feel so hideous right now. My nose is no longer slim and long like I liked and now it is short, wide and pig like with a point. Will the tip drop and look normal again once swelling goes down? Or am I going to have to get a revision done? I am so sad and have been feeling depressed to the point where I want to drop out of college and not go outside anymore. I have contacted my surgeon but I think he is ignoring me because I had an anxiety attack after my cast removal because it looked like not what I wanted. Can I send you pre and post op pictures for advice? What do you recommend? Any help is appreciated.

aarya nol you are only 7 days postop. You have a year until your nose is fully healed. Pace your anxiety. If you have a history of panic attacks or anxiety it is imperative that you work side by side with a/your therapist while you heal to give you perspective. Hello doctor I have a huge question hopefully you can answer it .. I just had surgery on July 29 it was septoplasty with rhinoplasty ! I did it with a ent doctor because i couldn’t breathe so he ended doing the jumó I had in the middle and now that the cast is off it’s straight when you look at me from the front but if you look at me on my side profile from the right you feel a slight bump still and on the other side there is no bump is that normal? Will that bump go away or does that mean he didn’t fully shave it down?

Had revision surgery 3 weeks ago (thick skin) and my left side profile looks short and just fine but the right side is much longer and it makes no sense to me how that’s possible, could this possible be swelling. Also nostril holes being two diff sizes. Any guidance on this would be so appreciated as my anxiety is through the roof.

@@sihammachlouch8055 yes if you’ve had a lot of work done there it does swell. I only had a slight shave - I taped and massaged a bit but I think the only thing that helped was time.

Hi Doc, on my 2nd week post op. Inside my right nose the septal wall is still swelling covering the air passage completely. What is the easiest way to reduce the swelling so I can breathe on both sides of my nose. Is a steriod injection an option?

Hi Patrick. At 2 weeks postop there should never be that much septal swelling that it blocks your airway. Please go see your surgeon to make sure that you don't have a septal hematoma. If you do not have a septal hematoma, then your surgeon can consider putting you on oral steroids to reduce the swelling. I do not recommend injections of steroid to the septal tissues because of the increased risk of a septal perforation.

Hello Dr Shah, I am 20 days post-op. I have more swelling in the tip of my nose internally which is producing a bit of discomfort/ stiffness on one side. One nostril seems bigger than the other. I don't know if this is due to my doctor bringing my nostrils closer into my face. Moreover, my nose has been getting more movement, thus, I have been feeling a few dashing feelings/ tingling around my face (upper lip, cheeks) is this normal? Thank you,

All this is normal. There is normal asymmetric swelling after rhinoplasty. The nerves are waking up and creating those feelings.

hi, i've just had a rhinoplasty - is your technique to managing swelling compulsory i am afraid to touch my nose. It's been 3 weeks the swelling on all points on nose are still sore

I recommend that you speak to your surgeon. My patients start this practice at one week postoperative.

Hi I had a Rhinoplasty 2 years ago and I’m somewhat happy but I feel that it’s still swollen. When I got my cast removed they said I was one of the most swollen pts they’ve had. I looked like shrek. It’s gone down but I feel that it’s still swelling especially at the tip and causes my nostrils to look a little uneven. Do you think I should get kenalog shots?

Taping might help, but only in concert with steroids. I don’t recommend steroid injections at this point. What I would consider trying is micro needling of the nasal tip skin no deeper than 1mm followed by the spreading of liquid Kenalog (steroid) onto the treatment zone. The little micro needling holes allow for gentle, even distribution of the steroid without the risk.

My patients wait about 2-3 months before reinserting the nose ring to avoid infecting the nasal tip. I would ask your surgeon for their guidance since they know the technical details of your procedure.

Hello, Dr Shah. My surgery is in about 3 weeks and I have a question, people say different things all the time about this so I wanted to ask you. If my surgeon doesn’t tell me about taping should I do it either way for my sake or should I just let it be? Im hoping to reduce swelling as much as possible so tips would be absolutely amazing.

I got my internal splints removed today which took quiet some time for the doctor to remove it and the amount of pain it gave me was unbearable, my nose got super swollen after that and then the doctor put external splint on the nose afterwards and he told me to come after a week for the external splint removal, right now i am so worried about my nose, will my swelling go down after i do the taping and the exercises you told in this video?

Some patients benefit from an extra week of splint usage. Once you remove the splint in one week, taping and gentle lymphatic massage as I describe will help you reduce your swelling in the long run. Every patient has different issues that will determine the amount of swelling relief they see over one year. Please discuss your particular situation with your surgeon.

Hello Dr. it’s been 2 months and one week since I had my rhinoplasty. I been getting steroid shots every month because my doc said it will help with the swelling. And I feel like after the steroid shots, my swelling got worse and the tip of my nose is so swollen like if there’s a ball inside. Is this normal? He didn’t tel me to massage or anything , what do you recommend? Thanks in advance :)

It should help. After steroids I recommend taping to control the recurrence of swelling.

@@anumasim6891 injections are trauma. You’ll have some increased swelling but should shrink starting around 2 weeks after an injection.

My daughter is 9 months post op to reduce a small hump and slightly refine the tip. Her nose looks great, so natural that family and friends don't even notice she had surgery. But there is no supratip break and we aren't sure if it's still swollen (she has medium thick skin and her nose still feel firm and numb) or maybe the surgeon didn't make one? She doesn't want to insult him by asking about it, which is crazy. Her Dr. doesn't like to do steroid injections. She wore tapes at night for 3 month post op, but never massaged her nose. She exercises a lot, so I know that can increase swelling. The last time she saw him was at 4 month and he will see her again at 1 year. Should she start massaging her nose now or is it too late? Also, can you use your fingers or do you have to use the tongue depressors? Thanks!

Actually I’d just leave it alone at this point. The last 20% of her swelling will go over the next 3 months. Let it proceed naturally. Not all noses need a supratip break that is obvious to the casual observer.

Once it’s out, I couldn’t put my in at all, your nose immediately gets swollen once the cast is of. Also you can’t massage it or wash your face properly with the cast. I found tapping and massage to work extremely well for me in the week following my cast removal
