If you have clients who have ice-pick-type pain but you can't reproduce it in the levator scapulae or rhomboid, consider the serratus posterior superior. This tiny muscle is hidden away under the scapula and often develops dysfunction in throwing athletes, desk workers with poor posture, and tradies who do a lot of lifting.
The trigger point is quite difficult to reach as it's nestled right under the scapula. The trick is to have your client reach as far across the underside of the table as they can to really bring that levator scapulae into protraction.
The serratus is a very small muscle, so be mindful of your clients during treatment. You won't need wide gliding techniques here as they just isn't enough surface area. Instead, use slow, small sweeps with your upper arm as you rotate your shoulder.

This couldn’t have come at a better time, I’ve been scratching my head for possible leads, when rhomboids, Lev Scap and Splenius Cerv. have not worked.
