I personally think this is one of the best bromance and touching scene in film history... Jack (Crowe) gives up his lifelong dream and sense of duty as a military men to give his dying friend Stephen (Bettany) a fighting chance to survive... I'm sure Stephen not only wanted Jack to stay to help him with the surgery, but also having the presence of his dear friend by his side gives him strength... Just a beautiful scene in a beautiful extremely well-made film.

There is also the in universe practical and military reasons for doing this. Morale is low and the men need a break, he exceeded his orders to follow the Aceron as far as Brazil a long time ago, he has no guarantee that the Aceron was the ship he sighted, and regardless of any other thing that may happen or action he might take, his only good physician is seriously wounded, meaning that even if he just turned around and made all speed to Portsmouth and did nothing else, he was jeopardizing his crew. To say nothing of leading them into battle.

You're right. With no understanding for the source of infection (bacteria), in that era abdominal surgery was typically only done when the situation was life or death. Surgeons dealing with battle wounds to extremities like arms and legs often went straight to amputation because the risk of infection was so great that simply removing the bullet did no good.

I performed surgery on myself. When I was in elementary I had what I thought was a spider bite on my right butt cheek. But it was an abscess. It was huge and puss started coming out whenever I put pressure on it. It also hurt very, very bad. One day when it got soo big it looked like a huge swollen nipple, I took a very sharp knife, stood on the counter of my bathroom sink and bent over. I poked the nipple looking thing with my knife and just stabbed it through blocking out all the pain mentally. It took a lot of strength mentally to get myself to do it. Then blood and puss started pouring out and I applied pressure and it seemed to never end. After I thought everything was out I had soooo much cleaning to do and then turned on the hot tub and got in. I was really proud of myself because I was like 12 years old or something and that thing bothered me for a very long time.

Adrenaline and excitement (from hunting the bird) plus laxer gun safety training probably played a huge role in the incident. It’s possible he wasn’t as experienced with ship and rifle doctrine as his subordinates.
