I usually wear L gloves, but for starting an IV, I wear M gloves. The tighter fit gives me better tactile sensitivity while still keeping me protected.

depends on the temperature, my biggest issue is flicking the iv to slide it especially if the skin is a bit grippy

Good tip! Thanks! I wear large as well and usually rip off the tip of both my index fingers but I usually end up with blood on those fingers. I really can’t feel with gloves on & I’m really skilled wkfh IVs. I will try the medium next time and see if it helps me as well. Thank you!

I've only had to start a few IVs in the lower extremities over the years. (RN 36 years) We needed an order to be allowed to do this. I'm old but I still like to review information and see if there's anything new under the sun.

Thanks for the info. I really wish I had gotten some hands on training in starting iv access in the foot and ankle as a nurse. I remember getting them as a little boy in a critical care unit sick with diabetic ketoacidosis and how much they hurt from multiple failed attempts. I was surprised that even at the infusion center where I get ivig & iron that the outpatient staff did not do these when I had multiple failed arm / hand iv attempts as an adult prior to getting a power port central line implanted.

if he was going to need several days of IV and was so difficult getting a cvad (i assume this is long term if they chose a port) would probably be a good idea they should have jumped on sooner. Poor kiddo :(

outside of theatre there are only a few cases where this was necessary on a ward (similarly to hands) this was painful to watch. the idea of determination and confidence is crucial to success thanks for reminding us again. Dont forget if you are looking for veins, even larger people have a fat one on their shoulder

Yeah, that saved me once in a patient whos lost his jugular access in the middle of a hip surgery. Detail: his both arms were bandaged and hurted in the accident too… his neck was enlarged due to the extravasation of iv fluids and it was an unpleasant position to be, it was that or subclavian vein in an obese patient in the middle of a trauma surgery. To help, the patient had respiratory obstruction becouse if the cervical enlargement after extubation and was difficult airway🤦🏻♂️, needing an emergency supraglotic device placement and fiberoptic trough intubation and subsequent, guide tube exchange to send hit to the uci with a normal large Et tube… those 2 hours destroyed me. Thanks god for helping me in sucha demmand higher than my own abilities.

I would love to see that, too, as well as for swollen arms or tough skin from venous stasis. If I remember it correctly, I have seen those started using an ultra sound device.

with ultrasound if you are a doctor or with a crazy angle and a long IVC if you are a nurse. For tortuous veins you can try straightening by hand and/or flushing to advance the IVC if you are already reasonably in (provided this is not a fragile hand vein)

Furing my younger days in 80s n 90s we used leg veins too for difficult cases n emergencies, even saphenoue vebesection, but after invention of different models IV cannulas the need to use leg veins is almost nil for last 25 yrs , but occasionally in severe shock like trauma, we use femoral vein to infuse fast fluids in ED , which can be removed after pt improves to avoid future complications.

Ive had a time where i started iv and couldn’t palpate properly, so took sterile gloves off and cleaned my fingers with chlorhex 2% soo i could palpate

Is it odd that I’m terrible with butterfly needles for labs? I’m an ER RN and I will always choose to do an IV to get labs and leave in place until discharge over using a butterfly. I can get an IV on majority of people; and I mean really sick, dialysis, sick cell, lupus, etc but for the life of me cannot seem to pull blood with a butterfly lol. Anyone else??

I struggled as an ER nurse with butterfly blood draws. There’s something to say for the flexibility and stability of an indwelling catheter that is inserted to the hub versus the floppy and unpredictable gyrations of a butterfly needle. I’ve had better luck with them when I have inserted them into the vein a bit more than I have thought was necessary so that the butterfly wings on the needle are able to stabilize successfully against the skin.
