Your story is somehow similar to mine, I'm a dv 2023 winner, currently in dubai, I decided to go to dubai after trying to apply for the greencard for more than 6 years without being selected.i got selected when I least expected. But I thank God I didn't lose hope, gladly have remained with 2months to finish my contract, then I'll travel back to kenya for my interview

You’ve got to be very lucky to get selected. The odds of winning are very low 0.67 or less in most years I don’t think it had anything to do with photos.

Bad photos can lead to disqualification so yeah photo has everything to do with not being selected

I’m not sure about this, what I can do if I were you is to fill the correct information on ds260, so incase they ask about being widowed then you have the death certificate,, you can also tell them it was a confusion mistake on initial entry if they ask
