Hello mate, i have a question in regards to one of your points made, that perhaps was mistakenly made. You said that c-pap is non invasive and then you classified B-PAP with ventilation. You said that B-PAP (OR BI-PAP) is invasive. Was that just a mistake? I'm guessing so, bc I'm sure you know, both c-pap and B-PAP are noninvasive. While yes, Ventilation is invasive.

Thank you for this lecture! You're a lifesaver. I'm just confused about one thing though. You mentioned that AC and CMV are examples of continuous ventilation and that the machine does the work to breathe. However, at 9:54, you stated that in AC and CMV, the patient does the work.

The machine does do the work with AC ventilation. However, you can set a back up rate so if the patient ends up not achieving the rate set in the machine, it will trigger the breath. Watch out for tachypnea which you can see in a COPD patient because it can lead to air trapping

thank you so much for this. one question: you said a CMV does not allow spontaneous breathing (i.e the machine does the work of breathing), but when explaining AC/CMV, you said that patient does work of breathing? also another thing is that when triggering a breath at AC mode, how is the patient able to drop the PEEP into a negative pressure, even though we set it to 5? shouldnt the breath be triggered from the PEEP we set? dropping it to negative for inhalation is causing the alveoli to collapse, which we dont want. can u please explain this?

Thank you for this video because it's clarified a couple of things for me but you mentioned that AC or CMV is mandatory mechanical ventilation but also mentioned at 9:54 that the patient does the work of breathing. I've read from a couple other sources that the patient CAN attempt to initiate spontaneous breathing but in CMV/AC, this is overtaken by the machine so that the patient isn't actually doing the work of breathing but they can initiate if they are capable. The machine takes over to complete the breathing cycle.

30:40... i dont think we increase PEEP to decrease venous return in a failing heart, in fact, cardiac circulation is compromised as diastole is compromised if we increase PEEP. but the interstitial fluid accumulation will be prevented from back pressure changes. great lecture!

20:20 to understand it, imagine putting a barometer inside trachea. Inspiration : air goes rapidly in by venti, V constant, P adjusted automatically via venti, to maintain steady volume flow venti force air into lungs, now imagine barometer going up, now after lung opens it no longer resist expansion (imagine balloon -> first blow is hard then easy), so flow is steady, but as volume built in pressure still goes up but not quickly as before so that explains why curving to right at peak, also explain platu and decline of air flow. Now during expiration air goes out but remember barometer doesnt measure direction so it doesn't matter air go in or out if there is more air -> more pressure. So air go out flow reversed (-ve increase), pressure decline as air is getting less in total, now when air is at PEEP, means there is still air but very few. Now if venti start pushing air in again new cycle start, if patient start spontaneous breath (hering reflex prevention of collapse) chest creates -ve pressure it will be reflected on lung -> trachea -> barometer -> -ve. Machine understand dude this guy wants to breath, let give me hand, it will start new cycle, no need for chest to do active work, PPV maintained. But we did let little -ve to build up, if we dont want this at all, set venti to fully control mod with PEEP 5. Now there will be no way lung can collapse.

When pressure increases and volume remains the same, it means that the volume inside is unchanged because the machine is doing extra work. Therefore, it requires more pressure to overcome the resistance from the lungs, which implies decreased compliance. This decrease in compliance might be due to sclerosis, edema buildup, or other factors.

21:52 to understand this, Inspiration: barometer ↑, more resistance → more work by venti → peak ↑ Expiration: lung contracts → pressure plateau (↓ air → ↓ pressure + lung compliance → ↑ pressure => plateau) Any resistance will peak pressure and increase plateau. As lecturer says, airway obstruction affects peak more and compliance to plateau.

thank you for this wonderful lecture. just wanted to ask something, at 22:25 you have mentioned that peak pressure increases with increase in airway resistance but the text says otherwise. are you trying to refer to general airway pressure in the text or does it refer to the peak pressure?
