To ERR IS HUMAN. NOTHING HAS CHANGED. WE ARE PRACTITIONERS...WE ARE NOT PERFECT. IF we do not learn how to identify the reason for errors and instead focus on the error itself, we can't decrease the risk of harm. Nurses are not demonstrating their knowledge of root cause analysis but are focused on punishing people for making mistakes that could be prevented if they really cared!!! Patients don't even have wristband on to identify them or their code status. We gotta do better.
