First pregnancy is not always safe for an Rh negative mother when carrying an Rh positive baby because her mother may have been Rh positive and at her birth would have developed antibodies due to the mixing of her blood and the mother's blood, and also the Rh negative mother may have had a fraternal Rh positive twin inutero.

All Rh -ve mothers receive (Anti D) during pregnancy 28 wks and within 72 hours from delivery whether they are sensitized or not. Perhaps the dose of anti D injection may differ according to the sensitization level by the Kleihauer Betke test or clinical condition such as abortion or abruptio placenta or .....

Ahmed Bayoumi my first and only child was Rh negative so I never received the shot. Second pregnancy was Rh positive and my body rejected it from the start. But wasn’t from antibodies from my first child as she was Rh negative. So my doc opted not to give me the shot. It wasn’t necessary.

Ahmed Bayoumi agree. I was lucky. My daughter was born in 1993. Father carried the trait and I am A-ve. It wasn’t my daughter that sensitized me to the D antigen because she was Rh -ve. Not sure where I got the antibodies from. It’s possible I suppose to be a twin of an Rh positive or something similar to acquire the antibodies prior to or at birth. But it wasn’t my first child who sensitized me. I was lucky and blessed she shares my blood type.

When mother is type A with a genotype AO, while the fetus is type B. Isn't it also a scenario of ABO incompatibility?

Yes, mother genotype is not important here, mother AO genotype gives A phenotype (anti B antibody). Mother type A, fetus type B. Fetus blood B antigen entering mother circulation is destroyed by mother anti B antibody.

In ABO incompatibility can't the mother be A ( or B) and has IgG ( hemolysin/agglutinine) (Ig M that switched class and became Ig G after a wrong transfusion /vaccination..) and the baby is B ( or A) ?

Eden At War Anyone with O- is RH negative, hence the - sign... anyone with - in their blood type, A- , O-, AB- &B- are all RH negative. Whereas, if you have type O, A, AB, B .... there is no Negative symbol so they are RH Positive... Is that what you were asking?

In my experience, they didn’t bother to test for the RH factor until I lost my first pregnancy... and even then, I was a couple weeks into my second before they bothered.. and I received the shot.... when my daughter became pregnant.. I told her to ask to be tested if she was rh negative or not...and they said that it was so rare , they didn’t bother.. that is why I believe that Type and RH factor are completely different things!! I know my blood type.. and so does my daughter.. but the Rh factor is a whole separate thing..

It's not possible. If mom is O then the baby can only be O A or B. That's because to be type O both your alleles have to be O. This means mom only has an O allele to give to baby.

Yes, it's possible. Since Rh positive is dominant, you only need 1 positive allele to be Rh positive. If both parents have 1 positive and 1 negative allele, that's a 25% chance of an Rh negative baby.
