As it is aortic dissection, beta blockers are typically the first-line treatment to control blood pressure and prevent reflex tachycardia. Nitroprusside may be used in addition to beta blockers if needed to further control blood pressure.
@DrElla-vm9mo4 жыл бұрын
May be B blocker🤔, I think using VD first will cause rebound or reflex tachycardia which it will exacerbate the condition 😅

start with B-Blockers; if we start with Nitrates we'll have our heart beating fast and a little pre-load (little time for ventricular filling). we'll then have a little stroke volume. I don't know, correct me

Thank you for this. I have an Ascending Aortic Aneurysm 4.5 cm and pain in my chest and back and they are taking their sweet time getting me an echocardiogram or TEE even, I am on zero meds, I found this because my Dr wasn't getting me in for a chest CT, my arteries in my neck feel swollen even and I am nauseated and sick every day, OH I PAID for the chest CT as I wasn't going to wait (Canada) even yet another month, and voila...I was right, it's serious, I didn't even say I told you so, didn't get the opportunity, as my GP informed me BY EMAIL and now I am waiting for the Aorta Disease Clinic to call, but I feel like we should move umm....faster? My Dr just sent me an email saying she didn't know much about it and to take a low dose aspirin and let her know if I got a nose bleed? what EVEN IS THAT. Sorry to go on here...just this was a good amount of info, as I don't understand the waiting for a full on dissection, what If I've already had on or the flap thingy etc etc (dammit Jim I'm a musician, not a Dr.) I've seen lecture after lecture saying that many dissections of the ascending aorta happen

My friend just had an aortic dissection and he is barely 40 years old, is physically active and not obese. I'm trying to find out why this may have happened to a person so young and I'm not finding any information other than Marfans (which he doesn't have) or EDS (which he also doesn't have). Any other possibilities that come to mind, anyone?
@MedicosisPerfectionalis3 жыл бұрын
I am sorry to hear that! Other possible causes of aortic dissection include: Copper deficiency (weak vessel wall), Hypertension, Coarctation of the aorta, Bicuspid aortic valve, Trauma, Cigarette smoking, High cholesterol, Atherosclerosis, Loeys-Dietz syndrome, Marfan and Ehlers-Danlos, A Familial disease. Hope this helps! Best of luck to both of you!
@L_MD_3 жыл бұрын
Patients with Turners syndrome are at risk of this if there’s a history of bicuspid aortic valve and or repair of coarctation of the aorta.

Hit the RCA (wether an occlusion or as an Aortic Dissection complication or etc,):possibilities: Inferior STEMI. Mainly RV and diaphragmatic wall of LV. SA node,AV node ischemia.(leading to arrythmias) Right atrium ischemia. IV septum infarct also.(about this one correct me if I'm wrong please)

We definitely can't downplay the possible urgency of an AAA. At the age of 23 I was DX with a stable dissection (it had happened previously), an AAA, cushings syndrome and Conn's syndrome. After one full and one partial adrenalectomy (then becoming AI) we discovered my AAA had grown large enough to warrant a large open repair with a Dacron graft (age 26). After sepsis (mutilation of 3 of my ribs because infection ate them), adrenal crisis and a spinal cord infarction, I narrowly escaped with my life. The damage to my body is irreparable and I still have a smal type b anneurysm because it was too large to repair. It's my understanding I'm more graft than aorta. I enjoyed your video. Thank you.

You’re welcome! I am so sorry that you went through this...It must have been hard! How is your health today?
@Mobycat264 жыл бұрын
@@MedicosisPerfectionalis thank you. My health is actually pretty poor atm. The aldosterone tumor has recurred on my partial adrenal and I also have essential htn. I actually have a question you might be able to answer...as someone with Conn's would I have a larger or smaller amount of ACE2? I'm being treated with eplerenone does that affect the answer? Lastly if the answer is a greater amount, could covid19 be more virulent to my system? I'm already immunocompromised because I don't make HC and replace my steroids. I ask, because most docs find this stuff interesting and I'm genuinely concerned.
