Sir , with due respect, for empirical treatment I would avoid ciprofloxacin due to latest guidelines and instead put the patient on Norfloxacin + tinidazole combination. Works all the time with excellent results. Great video , explained very simply and clearly.

If patient is having abdominal pain with diarrhea avoid loperamide because loperamide will exacerbate abdominal cramps and pain. Use loperamide in watery diarrhea with no abdominal pain.

Arsenal dream, you may start the empiric therapy with Cipro and metronidazole with fluid resustication. And if the patient is experiencing abdominal pain. Releive the pain with antispasmodics like drotraverine, mebeverine.
