Dear, This example is to explain splitting process. And quad mesh...if any of region subject to high stress (stress concentration)...that need to mesh very fine... washer may be useful tool then.

It dependents. For structure analysis no issues...but these elements are not recommended for dynamic analysis in which wave propagation take place.

dear, for crash analysis mesh flow is more important for better convergence of solution and wave propagation, so we accept tria element there up to certain limit [usually 10% or less].

Dear, Minimum length quality is important in case of explicit dynamic solver. If one want to perform static structural simulation... minimum size not an issue.
