Do u have to get madness while wielding the madness hands or does the buff work when you got them in the invetory equipped?

Yes, you have to equip it while inflicted madness. Buff will only last 30s. You can swap black dumpling after madness inflicted but I did not test to swap madding hand as well.

What is the reason it is so busted? Is there a specific?

Cuz it's heavy attack has two trusts and royal knights resolve applies on both trusts
