Excellent video. I have watched many videos on this subject and yours is the only one that tells how to feel to find the joint, then place the pad right behind that joint. There is tenderness on that joint, however, as I feel further up towards the toes it gets more tender (painful). Is this typical, or does it indicate a different problem/solution? Thanks so much!

Hi I am glad it was helpful. The dome could still be helpful. However it is important to find out why the pain it present, then you can build a plan. The dome may off load symptoms but will not tell you why the pain is present

@@NKActiveClinics My history is that I have a non-malignant tumor in my 2nd toe, and as it's easy to break I've broken it 3 times. Now I'm more careful! Each time I broke it, it caused capsulitis, swelling in the joint below the toe. The toe healed, and the capsulitis went away. This time the X-ray shows no breakage (and still no growth of the tumor) so we don't know what's causing the pain. A cortisone shot didn't help, and the next step is an MRI to see if surgery will help. I wouldn't consider surgery without giving it plenty of time to heal by itself, as it has in the past. But it's not getting better as quickly as it did before, so I thought I try the pad. Thank you again for the excellent video, and your reply.

@@peterchristensen9585 Thanks for the reply, this is a complex history, in theroy a metatarsal dome could be helpful, however I would chat with the person you are seeing already to discuss with them. The other thing to look at is toe strength and make sure you are working on building strength. We can offer video appointments if that could be helpful for you. Thanks Nick

Why do some people stick it under the orthotic or shoe lining? Does it have any bad effect to be underneath ?

Some people do not like the feeling of the dome on top. I tend to find people tolerate the dome better underneath.

@@NKActiveClinics Thanks for the fast reply...is there any downside to the dome underneath? I just worry it will change my foot posture or how my insole is working, Ive had mortons neuroma surgery and 4th toe straightened....but I get a lot of pain under my 2nd toe from too much pressure on it

@@IrishPharaoh2008 No worries. no insole will change you foot posture permanently. A dome underneath may not off load the foot as much. However my advise is to speak to the person who has issued your current orthoses if you are needing to change your dome position or about the effects of your surgery

How come scholl metatarsal pads are wider and slip onto one toe with ring ? Which is better. Where you buy those domes! In Australia. How long for condition to go away ? . What about gel inserts

Thank you for the questions. I will try to answer them one by one. The scholl metatarsal pads are wider, and effectively are just cushioning, rather than trying to offload, the reason it has a ring for the toe is that you physically attach the pad to your foot. In general, they tend to be very cumbersome and I’m not overly effective in offloading the ball of the foot applying a dome to and insole or trainer lining tends to be more effective. I am not sure where you can purchase them in Australia as a member of the public. However, most podiatrists will have access to them. With regards to how long it can take to go away, this purely depends on the diagnosis. There are well over 50 different reasons as to why the front of the foot can hurt using a metatarsal dome can be in offloading strategy and part of a treatment plan however, you need a diagnosis first, and then you can work out how long it will take to recover. Gel inserts, very much like the scholl, will help cushion, however, will not offload as effectively as a dome, slightly depends on what the treatment objective is. My advice is, if you are suffering with pain symptoms is to see a podiatrist in Australia to help formulate a diagnosis and treatment plan .

Hi doc, i have metatarsalgia on 4th toe of both feet. It is very painful to walk around the house so I put exactly same dome that you have shown in your video on my feet just below my pain area. Those i got from dr office to try. I put it in hoka recovery sandal for now . I just feel some discomfort but is it ok to put on sandal since i use this while i am at home? Hi Nick Knight thank you for all your very very helpful videos! Question: should the metatarsal dome uncomfortable at all? I feel I’ve got it close to the right place but the pressure on the edge of my metatarsals is very uncomfortable. Maybe it’s just something I need to get used to? I’m using a carbon fiber plate so putting the dome underneath the plate isn’t an option. FYI 2nd metatarsal plantar plate sprain here…my podiatrist has not been very helpful and wouldn’t help me with custom orthotics. He told me to place the dome ON the ball of my foot to help “splay” the metatarsals. I’m on my own here.

Hi CinephileChelsea, No it should not be uncomfortable. It depends on the material of the time as if you’re using on a carbon plate you will tend to find to using a slightly softer dome more comfortable. There is also the element of some people just can’t tolerate a dome even if in the right place. There are still other options with the plantar plate injury like footwear advice, custom foot Orthoses, strengthening exercises. @@NKActiveClinics Thank you for these comments. They are very useful for understanding how to get better pain relief. I found must off the shelf insoles with a metatarsal bump pads are placed in the wrong spot for me. Once I realized this I was better able to make adjustments to the insole and minimize pain pressure.
