Is there a reason you don't use tables? Also some of field selections seems quite long winded. It also seems very much focused on getting items back from a client, rather than being the client reviewing the documents- where multiple people maybe required to comment or accept/reject the document. In such a case we not only have to track responses but also whether a document is approved or still out for review.
