Miley had Reinke's edema. She had surgery to correct it and it gave her a more smokey, gravelly voice quality. She explained it during a conversation on the Joe Rogan Podcast.

⁠​alcohol doesn’t deepen your voice or affect your vocal chords it was her smoking that caused her reinkes edema, which is swelling of the vocal chords due to excess fluid collected in her reinkes space, as well as Miley continuing to use her voice after shows where she said the adrenaline of a concert wouldn’t let her go to sleep so she would stay awake talking and then later smoking and talking, which wouldn’t give her voice a break and caused her reinkes edema

Perfect example of her voice transition. She was such a sweet and beautiful girl with a stunning voice! But she wrecked her own voice through smoking and personally I think her whole career has gone down the toilet since she left Disney. But that’s just me! 🤷‍♂️ I used to love her.
