Ghostface is your friend, girlfriend, brother, uncle, ect. The second thing is that there’s multiple ghostface killers. When you think there’s one usually there’s more. You just don’t know for sure. And the third thing is that he’s meta. Practically fourth wall breaking. He knows all the horror tropes and is self aware of everything. Knows where you’re gonna hide or what a main character would do ect.

It's not about the brutality but the intention and his personnality. The vast majority of the kombattants either don't want to kill but are ready to do so if necessary or kill because they have a goal. Even the most brutal and ruthless like Shao or Omni-Man kill not because they like it (they likely enjoy it but killing in itself is not the goal) and the same can be said about Shang Tsung and Quan Chi who are too pragmatic to just kill people at random like that even if they are sadists. Heck, you could say that even Homelander, who is a psychotic bully who loves abusing his power, has sort of a pseudo better reason for his actions since he sees himself as above everyone else and so in his mind, there's nothing wrong about killing everything in his path for the sake of it, it's almost his right to do that. Ghostface on the other end has no specific goal in mind besides killing. Not only does he know his actions are depraved, he deeply enjoys it and wants more. He kills because he likes it. No superior goal, no twisted ideology, not even delusions. Plain cruelty for the sake of it.

If you don’t know his “lore” Ghostface has been like 9 people, but all have used recordings of Billy Loomis (the first Ghostface voice being modulated) they’re all essentially copycat killers. They used a changer to sound like it later, but they RARELY actually speak in costume. Hence the tape/module.

There are tons of people in the super hero genre who don't need super powers to terrify others. Take Joker and Batman for example. They're just normal humans, but their skills, equipment and personalities make them intimidating to those who, by all rights, should turn the fight into a one sided stomp.

Bane kicked his Venom addiction, so now he's just a really intelligent body builder. The Ventriloquist is some dude with split personality disorder, and Victor Zsasz is just a deranged serial killer who marks himself with tallies of his victims.
