Hi Sir. in the question regarding ( Adam in automotive project) I still not sure why the answer should be "B" not "C". if the question says what should Adam do next or how to resolve issue so I agree the answer should be "B" . but it was asked how to prevent like this scenario in the future ( no the mistake already occurred and he knows why) so in this case answer "C"
