You will get a letter in the mail from FMCSA for your audit call. Do this ASAP. It’s easy, they basically just go over your application to verify what you’ve entered.

You don't normally need a CDL with box trucks. There are some box trucks (over 26K GVW) that require CDL, and in a way if you had one of those you might be able to make more because you can carry more weight.

You will need to select the correct vehicle type. Amazon does not work with cargo van's though.

You cant run both household goods and amazon. Insurance companies won't write you a policy doing both. You would need to do a completely separate entity that is unrelated to the household goods moving company.

For an LLC you will need your Articles of Organization. It's safe to take a copy of a utility bill and drivers license with you as well. It can vary bank to bank.

Household goods is very complicated. In most cases, if you are doing household goods, then that would be the only commodity that you haul. Non-Haz Mat would want to be selected every time unless you plan on getting a Haz Mat license.

Should I still focus on "Interstate" Selection's even though I won't be going out of state for the time being or pick "Intrastate"? If you plan on running for Relay, you have to be approved for interstate authority to meet their requirements but you don’t have to actually leave the state and can just run local until you’re ready to go further.
