When he was told his condition was hardly noticeable, he was being lied to. He's aware of his partial transformation but has adapted to being half human and half alien. It's also not paranoia to constantly check for the parasite that partly transformed him. You never know.

OR he’s colour blind and as such asks if it is noticeable because he sees as a realy different shade of skin and veins. But since he is colour blind it might be him seeing it that way.
