Jimmy probably coaxed her into it, which is why when she's talking to him through the door she says "You were right all along, I should have done this from the beginning", and Jimmy knew what she was going to do, so he quickly diverted it to make it seem to Daisuke that she was trying to euthanize Curly instead. Jimmy never felt guilty over her death, what he felt guilty about was getting her pregnant, which is why the horse sonogram sequence

Jimmy doesn't actually have a thing for horses. That was him not taking the psych eval seriously. I think it symbolizes the fact he didn't view the child as something he wanted to deal with, but the third one being a human, one looking just like him, shows that it's something he can't deny. Deny the seriousness of his actions. Deny the repercussions of it.

Also, the fact that the ‘lips’ of that red blob opened up and released that mangled pony-looking child, terrorizing Jimmy, but never actually hurting him, shows how he viewed Anya giving birth to his kid if it were to happen. It kid technically wouldn't have hurt him, but it being born would've destroyed his life due to him facing the consequences of his actions. He viewed it as terrifying. He was scared of getting caught.

To simplify it Mouthwashing is about RESPONSIBILITY. The whole premise of the game centers around juryduties (Jimmy's) inability to take RESPONSIBILITY for his actions throughout the game. But not only that Curly's inaction and enabling of Jimmy's actions that lead to the eventual outcome of the game. I can go in more detail if you'd like. I don't want to spoil it too much incase you want to play it yourself.
