We just don’t know exactly how far ahead the timeline is. Also Kate went back to NY for a bit after relapsing because she assumed Tommy killed D-Mac when D-Mac went MIA. Tommy led her to believe D-Mac died but she didn’t know Tommy and JP sent him away. So when she was back in NY that’s when she got the letter from Diana. She’s not in Season 4 because she returned to Chicago and has been back in Chicago for awhile.
