He had mentioned meningitis and I lost a family member to that many years ago. The infection started in his ear and progressed quickly from there. There was a hole in his ear didn't get repaired. Once the infection came in he was gone within 24 hours. These ear cleanings can be a life saving for some.

I’m very grateful to be a meningitis survivor. I was only 2 weeks old and the doctors kept telling my mom it was only an ear infection and she was being crazy. The last test left was to test the spinal fluid. The rest is history (aka many many tiny IVs delivering antibiotics in quarantine). I wish more people were as lucky as me :(

I have watched other specialist in this field and this guy's approach is quite different. Dr Z use the proper tools needed to get in and out as quickly and less pain as possible. Maybe each country is taught different. This is like watching paint dry🤦🏽♀️

Dr. Z is authorized to use anesthesia; Neel is not. He’s doing his best for the permissions he has. Britain doesn’t let audiologists anesthetize their patients and he’s probably not trained to do so.

I think people are comparing Doctors because Dr. Zhao does use a different technique when treating ears. I also may be that Dr. Z uses anesthesia and different instruments to be able to quickly go in and finish the job .

Wow, I never realized how bad an ear can be damaged by an infection! I had an inner ear infection/pneumonia/strep throat all at one time when I was a teenager. I remember how bad my ears hurt, I never thought they could look like this inside. I was hospitalized for 2 weeks but the ENT specialist I had didn’t do anything like this. Maybe it would have helped the pain. Glad this guy had someone like you to turn to for help. He must feel much better and able to hear easier too. Great video, WW!!😉

My son needed ear surgery when he was 10 because an infection caused some of his ear bones to dissolve. He now has prosthetic ear bones (sorry, I don't remember the official terms). It was very successful. His hearing is better in that ear than the other.

Thank you for being thorough and aware that clearing as much pus, dead skin and debris will help quicken the process of medication eradication of what you couldn't remove. I think that we as watchers of earwax removal need to remind ourselves regularly that while we have learned a lot about the ear (I’ve never totally understood the ear and how we hear, no matter how many diagrams I labeled in school) we are still just watchers and just because it looks easy or simple does not mean it is. We don’t have experience with this and I know that aim not an audiologist. I have been disappointed in myself many times for starting into that thought pattern and have to stop myself because it is really easy to start thinking that you are an expert in whatever youtube obsession you’re indulging in. Also, I don’t think that any one of us would want a stranger to walk up to us as we are working at our chosen professions and have them berate us on how to do our job! That’s just ignorant, inconsiderate and down right rude. Here is the US south we would say someone’s mama missed something in the manners department and call them “Tackey” or “Common as pig tracks” bless their hearts.... I am so on my soap box right now so I will stop ranting now. P.S. What I have learned from you and Ryhes Barber is how to use my cheap Walmart otoscope to look and say yes we need to go to the doctor or no we don’t. I never knew that skin could overgrown in an ear canal and I certainly had never heard of a cholestatoma before. Now, I know the basic definitions. I still can’t tell the difference though because Ummmm, I’m not an ENT or audiologist! PSS. Bless this poor man’s heart (in the good way, not the way that means “We’re sorry that you’re stupid.” That had to hurt! I’m assuming that there is just now way to do that without it hurting since the skin is...ummm...zombie skin? It didn’t even look like an ear canal. I hope he is able to see someone soon and stop it from getting any worse. He is lucky that you caught it. The doctor’s around here seem to be under the impression that you can throughly exam a sore ear in 3 seconds....

My uncle recently passed. Though he had multiple medical issues, his passing was unexpected. Still, this was one issue he got treatment for a few years ago. He had an ear infection that was eroding into the bone. had to have surgery to stop the infection, but it also left him deaf in the one ear. Better a few more years though, and one good ear.

A lot of time depending on the severity of the damage there's never any improvement. I was pretty much this patient 26 years ago and after quite a few surgeries and now hearing implants I'm left with nothing but a nasty smelly empty ear with just basically an outer ear to look normal. I have to have CT scans every 6 months, monthly cleanings to make sure the mastoid which is all i have left stays intact so meningitis doesn't set in and almost 51 years of just having to learn to deal with major ear problems.

This actually happened to me - all the GP could see was gunge, and they have no way of assessing what's behind it. 99% of the time it's nothing and clears up. The 1% tho....I ended up in A&E, the specialist there put plugs in to keep the canals open and they just swelled shut over them. Finally ended up in a different hospital in absolute agony and the ENT specialist there had to see me every day for two weeks to clean it all up. Afterwards I was annoyed with my GP. I think the first ENT but should have picked up that this was serious (instead of making it worse 😠) but the GPs just can't tell and can't refer everyone.

I always find it funny and interesting how some people with no medical training try to tell the doctor what to do. I'm a nurse and I don't think some people understand why certain things have to be done. The ear is so delicate and if you aren't careful and do things correctly you can really cause serious permanent damage. Also this can be very uncomfortable for the patients. I do really love his videos because of the commentary. I like that he gives alot of information. Thanks for all your hard work!!!

I work at a gas station for a major oil company and it blows my mind how many people try to tell me how to do my job when they have no idea what they’re talking about. He must be in so much pain I hope he recovers quickly. That poor man l remember the pain when I had a threatened mastitis . I was 8/9 the pain is something I have never forgotten. At 71 you would have thought it would disappear in the mist of time. After two children the ear pain eclipses them. So prayers for the man to recover and be pain free. This was just absolutely mind blowing to watch. You do not realize how big that crater is until you said you could put a golf ball in there..........think about the size of a golf ball in your ear canal. That’s crazy. This patient is such a trooper with the pain he just gave been in. What a unique and challenging removal. The keratin almost seemed to have a texture like jello that is stuck to its mold. As for the ones who are armchair audiologists - I don’t understand how they think you should use a different tool. Wouldn’t one think you would use the best tool?!? Just ignore them. I had an ear infection I didn't know I had it quickly as in next day turned into mastioditis, bacterial meningitis and sepsis. I spent 3 days in a coma then woke up for around 4 hours only to fall back into the coma for a couple more days. I had a ear tube put in. Spent over 2 weeks in the hospital most of the time so stiff I couldn't move. Went home with a picc line for 8 weeks and then had a mastiodectomy. Yeah... all started with an ear infection

Hi Neel. OMG. the ear was very problematic with lots going on. To be honest I couldn’t make out the drum. I know you pointed it out but it was confusing. Three big cavities where the skin had eaten away the bones. The whole ear was a mess. Thank goodness you could remove as much as you could. You’re so gentle and always mindful of how painful it could be for the patient if you touched the walls. Very well done. You’re amazing. Xx

My son had a colestiatoma and we ended up at UC Davis and they had to remove all his small bones in his ear and put in prosthetic bones. It gave him back much of his hearing but many years later he can’t really hear out of that particular ear.

I'm always annoyed by people who leave comments on what you should do and what you are doing wrong even though you are a medical professional. I'm a well experienced and knowledgeable nurse and I understand that things may seem easier or to make more sense to someone without medical training but it's not always the case. There's always a reason for the way things are completed medically and that's why extensive training and along with hands on experience are so crucial. Your job is to approach and resolve the situation with the least amount of discomfort and avoiding any further damage or adverse effects. Your job is to help resolve not cause any further issues.

I know this poor man had a number of serious issues but what an amazing job you had on your hands here... the GP could give him any number of antibiotics but without that mechanical removal of all the pus and infected skin and debris it wouldn't have been for much. The pus and debris re- forms so quickly, and the erosion signs were a real worry. Pretty breathtaking work for all that you had before you, in a limited space of time. Ive watched this video more than any other! This is the worst ear infection i have seen and you did a fantastic job with it!

Excellent job Dr. Raithatha!! I cannot believe the damage to this patient's ear OH MY GOODNESS!!! 😢 The pain had to have been unbearable. I do hope they are doing well today

I learn a lot from these, I’m in the US and a physician we have a different system of course and don’t have the basic GP model , I’m a med/peds physician so did residencies in both internal med and pediatrics so have more training then the standard FP- family physician but still considered primary care- the tough part is while ENT would see this pt because obvious surgical need- they make $$ on procedures , and it’s expensive for a visit twice as much or more than primary, so generally I’ll be ordering the CT getting the culture start abx etc then when CT back get urgent referral, otherwise the pt will wait up to 2 weeks and if Medicaid longer! I may also direct admit if lots of bony involvement for broad spec abx awaiting culture result- then I can force a within 24 ENT consult regardless of insurance - staff requirements !

Ouch!! I can’t imagine how bad that had to hurt!! I don’t think people understand how dangerous ear infections can be, I know I didn’t!

So very interesting. How gentle you were with the patient. I imagine great skill is needed to get all the gunk out. Do you ever find out what happens after your referrals to doctor? Thanks for uploading and please ignore the ignorant and insulting comments people leave. I think you are a highly skilled and knowledgable guy.

So this is the right ear. How was the left? I thought 592 was the left ear but it’s not. Is there a video of the left side?

Of all the videos of this sort I have watched, and from several audiologists, this has got to be the worst case I have seen. I hope this guy gets better. I can definitely see how this condition could become life threatening. Thanks for helping!

Always makes me giggle when he says he can only do but so much because he’s not an ENT but they always send people to him. Seems that he should be given more privileges than he has. Especially since he’s a multi award winning audiologists and development of specific tools. Just saying. I love you so much but the way. You are definitely one of my TOP man crushes!!! Just saying again, lol. Love from the USA

Is it possible the pt had a deformed ear canal the whole time and the altered shape was more conducive to infection instead of the infection changing the shape of the canal?

I am not sure why you would pay any mind to someone critiquing you on KZbin. The fact that someone does is testament that they do not understand the complex nature of this job and procedure and how much advanced skill and patience it takes to handle these very tedious procedures. Unfortunately you almost make it look easy because you know what you are doing and what approach to take for each scenario. My guess is that the difficulty of clearing this ear rivals other surgeries and so really you should not in any way be bothered by such unenlightened comments on your channel. Thank you for you commentary and for educating us on these procedures!

Is there anyway to remove the ear hair? It seems certain people have allot of ear hair, and need to have it trimmed/waxed.
