Leigh attempted to replace Merle Oberlan in Wuthering Heights to be opposite Olivier but failed. When Leigh was chosen to be Scarlet in Gone with the Wind, producer David Selznick hired guards to make sure no one was sneaking pictures as he wanted his Scarlet “pure”. He asked his brother, the agent who was in charge of Olivier, to find something to get rid of Olivier. He got him a job on Broadway. In the last scene in GWTW, Leigh’s tears were real because she was forced to miss her flight to New York to be with Olivier. They eventually divorced their spouses and married.

I don´t know if I would count Angelina Jolie and Brad Pitt here, because you said it yourself it´s not verified that they had an affair while he was married. Yes you can have an emotional affair.
