Yep, it plagued the majority of his younger years. I remember watching a interview with him when he was the AnR for DefJam south and he was still suicidal at that point talking about jump out of the window to his death as an option. I didn't noticed a change until much later in his career when he seemed to find more comfort in religion. Seems a very different guy these days.

This song addresses mental illness in the hood because it is something rarely talked about it’s as if the world thinks that people in the hood are just rotten people, but if you ever grew up in the hood, you know that the things people do, cause all types of mental illness. When Ryan says why is his mind playing tricks on him, he is saying what is not said but understood, about a hood dude going thru it because of his “hood” dealings hope that helps

If you only liked songs that had lyrics you could relate to your own life, I doubt you would like a lot of metal music. Like I love hallowed be thy name. But I have never been awaiting death in a cell, about to be hanged while contemplating God/Death/The Afterlife. Similarly In the song Represent by Nas he talks about how on the mean streets of Queens Bridge he and his posse keep guns for anyone who looks at them wrong, or out of town folks who come around poking their noses where they don't belong, and for dirty cops. My life doesn't at all relate to that. But I love the song. I think that is the beauty of music. I can be introduced to things I've never thought about or I can listen to stories about experiences I will never experience. While appreciating the music that was made to accompany that message. I hope you can do the same.

I like to think of this RAP as an ALLEGORY of mental illness. Each of the Rappers talk about a certain personification of the serious mental illness and paranoia that comes with that life of a drug dealer and what happens with the cops or the people that they might have wronged. This song is actually about addiction to crack/cocaine and the effects it has psychologically, the drug was very popular among black urban folk during the 1980’s & Early 90’s, which is when this song was released. But it’s also subliminally about Scarface and his personal psychological issues in his life.
