Definitely took about 3 weeks to build up to nightly. I made the mistake of getting it too close to the corners of my mouth. I was on a ventilator for about a month one time, so I’m already prone to that area cracking. But I had to keep it away from that area for awhile too. I’m about a month in and my skin has adjusted beautifully. No dryness anymore 👍🏽

A very intelligent approach. I wish doctors would have provided this kind of information 25 years ago when I first tried Retin A. I live in a dry climate and without conscientious maintenance my skin is always dehydrated and often feels like it is falling off in winter. Knowing that I could have used a moisture sandwich when using a retinoid would have helped greatly. I gave up on it as my skin felt like it was drying up so badly. Ironically, I have used retinols for years with no problem, but I always added a layer of moisture over them. My dermatologist years ago suggested doing the same with a retinoid would make it ineffective. So much for that notion. I am thinking of trying tretinoin again so I am gathering all the info I can before I set off on the journey. By the way, I ordered your sun screen. It is wonderful, especially in a dry climate. I plan to stock up before winter. I need all the moisture I can get when the Mercury dips to -30 C (or -43 C as it did this past February!).

I just moved up to 0.1% and to be safe, am moisturizing first, with a fairly heavy moisturizer.... I’m at about day 4, I noticed some small areas of peeling/dryness. So even over a thick moisturizer, Tretinoin cream works. I have read some studies that say it penetrates easier and works better on moisturized skin.

Dee Wye I actually recently read an article that said a lot of dermatologists think eye cream is a scam. I was shocked and now am suspicious of all my pricey eye creams. 😭 Apparently many believe that if your moisturizer or other skin care products have good enough ingredients for your face that it’s good enough for your eyes too (unless it says not to use around your eyes).

If you already have a good all around skincare rutine, there should be no need for it. Some use eye creams religiously, and swear that it makes a huge difference. But I honestly think eye creams are a huge scam, yes there are good ingrediens in some, but there are the exact same ingrediens in some of your other skincare products to. I myself is soon close to my 40's, and talking with other people at the same age, I hear a difference in rutines. Like a lot of people rub hard on their eyelids and eye area, when they remove makeup and do their skincare rutine, insted of just treating that area carefully to prevent earlier wrinkles. Like not touching your face all the time (have been a habit of mine), also gives less breakout, so I personally think a good skincare rutine and thinking about what you do to your face, can help your skins condition for the rest of your life. That ofcouse does not mean I am right, but it works for my skin, and I save a lot of money.

A good eye cream has the same exact ingredients as a good moisturizer. “Good” meaning a mix of humectants, occlusives, and maybe some antioxidants, and NO fragrance or essential oils or citrus. If you already have a good moisturizer, it is more than efficient for your eye area. The only time you would use a separate eye cream is if A. a prescribing medical professional recommends it, B. you want to use a targeted active ingredient like retinol or phytic acid ONLY in the eye area (which in this case, you don’t need to find a product specifically advertised for the eye area. You can use a face serum with hyaluronic acid *just* around your eyes; no one is going to break into your house and arrest you), or C. you use an SPF eye product in the morning for added protection.

This video is a GOD SEND. I'm on day 4 of using clindamycin, benzoyl peroxide, and tretinoin and my skin was drying out like crazy. It actually burned to put moisturizer one especially around my eyes. I definitely gonna start moisturizing before and after, using moisturizer goggles, reducing frequency of application for now. Also never heard of the fingertip measurement technique or 13 dots so will definitely be using that!

I purchased a prescription Tretinoin online a year ago and started using it but quickly went from perfect skin to dry skin badly. I stopped using it completely and did a lot of research then decided to see a dermatologist bc $260 for a tube was stupid. So she called me in a prescription and though it was still $250 I was impatient with insurance so used a discount card and paid $59 and started again tonight. I’m 34 with 7 kids and generally super healthy skin. No bumps, discoloration, etc. but I Was prescribed that and benzyl peroxide with Clindamycin, for one spot I randomly developed on my forehead a month before I got pregnant with my 7th baby and stayed even though she’s now 4 mos old. So I am hoping it helps make that go away and also with anti-aging :) thanks for the tips

I use Cereve skin renewing cream serum first. It has niacinimide in it and it’s super light weight. I wait around 30 min then apply my Retin A. I use to use it without a buffer and my skin never adjusted. It was constantly peeing and looked terrible when I wore makeup. This way has worked so much better for me.

Dr. Sam, I know there are people like me with rosacea which have heard about using retinol. Nowadays, there seems to be two perspectives on the procedure to follow. One is doing it very slowly to avoid irritation as much as possible. The other one is going strong, no moisturizer, to get a short, but acute irritation in order to prevent chronic irritation. I'd love you to adress this topic, because my face is getting worse, but I don't know who and which way to go. Thank you.

I've personally heard a Spanish dermatologist, who also thought irritation was a big no-no for rosacea, until she received training from Dr. Obagi in CA. She herself had the most reactive skin, but decided to take this approach and succeded. Of course, she now sells Obagi products at her clinic. I'm both hesitant to be pushed into buying a specific product and also ruining my skin.

Lucía García Well, thats troubling that a dermatologist is going around saying that. Hopefully Dr. Sam will answer soon. If not and while I’m thinking about it, you might try Dr. Dray’s channel. If I’m remembering right, she has had eczema in the past and uses Retina-A regularly. Seems unnecessary to me to suffer when there may be ways not to. I know rosacea is different from eczema, but there could be similar challenges in having them and using Retin-A.

Well turns out my neck has zero issue with it, as does most of my face, I was however shocked how bad my chin area reacted to it...so I had to completely discontinue the use in my chin area just one little dab of it there and I looked like I had contact dermatitis 🥺🥺🥺

I love the cleanser, it’s ideal as it’s fragrance free, non-comedogenic and doesn’t strip the skin, but still removes makeup. I bought the flawless serum in September 2019. Unfortunately, I always had an event or an interview coming up and I was afraid of ‘purging’ /peeling / dry patches etc so never got round to using it. In the current situation where I’m less likely to see people, I thought it would be the perfect chance to start to use the flawless serum. I would like to confirm the consistency / use by date (on the bottle it has the 12m logo, which I think means it lasts 12 months once opened). When I used it, there were small ‘grains’ or lumps in the serum, not loads, just a few and I just wondered if this was normal/ok to sill use?

Also, what if you decide you can step up your retinoid and find out that it's too soon and you have irritation that's bothersome but you don't want to stop? Do you just return to the routine you were doing that wasn't as bothersome and then re-evaluate in another two weeks?

This was so helpful thank you. After a few weeks of using my retinoid product I’ve noticed my ‘marionette’ lines are more pronounced and I’ve panicked! I’m thinking that the product might have pooled up in the slight creases there. I will give it a rest for a few days then start ‘buffering’ as you suggest and take more time to smooth the cream in. At night I use tretonoin 0.05 I do have rosacea on the checks , come broken capillaries and redness, not too bad. Would this routine be ok for someone who’s not sensitive to retinoids but tends to have flaky skin on chin and around the eye?: Cleanse, niacinamide serum, prescripted retinoic acid cream, mositurizer? Since my mom already applies a 10% niacinamide serum before the retinoic acid cream and she’s not that sensitive, I assume that’s enough to “prepare” the skin. And the moisturizer at the end would prevent flaky skin

I know it's tempting to put it on your fine lines or around your eyes but at the beginning especially its not worth it! Rushing will damage your skin, and you will have to take a break and then work your way up again (which in the long run actually takes longer). Small amounts, away from your thin skin and slowllllllyyyy build up. It stops you from getting peeling.

I started with The Ordinary Gran. Retinoid 2%, then worked up to 5%, then switched over to Differin which I am doing right now. If you have sensitive skin, it's easier to ease into the strong stuff. A long time ago I did straight into Differin and it felt like I fried my face off!

I dont use eye creams they are so expensive & such tiny amounts...use whatever moisturizer I am using at the time or my go to fave CERAVE moisturizer 19 oz.tub great after showering & skin is wet lather it everywhere you can reach & no toweling..buy it on ebay much cheaper than anyplace its the cheapest I have ever found ......also follow Dr. Dray she has great info also

this only applies to dry sensitive skin. moisturizers contradict the reduction of excess oil and comedones build up.

Thanks for all these informative videos, these tips have definitely helped me adjust to epiduo better than I would've (although the purge stage was still awful but I dont think that can be helped)

Sarah Nelson A purge stage is a false idea. None of the reputable and truly knowledgeable KZbinrs condone that idea. Slow and gradual is the way to go. Dont subject yourself to a purge stage. That is truly wrong and not necessary.

Yes, I wash my face and towel dry it. Wait for half an hour and then apply tretinoin. Moisturise after 1 hour and go to bed. Love it.

hello Doctor , I am 29, male, I played a lot in the sun for last 10 years as an athlete and did not care about my skin, did not even apply sunscreen or moisturizer. BUT since last year i have started doing all that, my skin look so DULL and lose and under eyes look puffy and lose, smile lines. I applied retinol or last 3 months and it helped a little bit. Should i start with .025 tretenoin. how can i get rid of pufy and droopy eyes, will tretenoin help me

Monica Newna I think the goal is every day eventually, depending on your sensitivity. My understanding is it takes a long time to show effects, no matter the frequency. I hope Dr. Sam will answer your question. If not, Dr. Dray has a whole series in Retin-A, as does Hot & Flashy. I would think twice a week is a good frequency.

Hi there...i think that using two exfoliating products together is not advisable, it might be too irritating for the skin...i use glycolic acid in the morning and tretenoine at night.

I use acids and retinoids on alternate nights. I’m not keen on using actives in the morning, even though I always wear SPF - I treat it more as time for hydrating and protecting the skin 😊

My god I wish I knew this 18 months ago. I went from 5% Retinoid to 1% retinol because 5% was "too strong"...I didn't know the difference between "granactive retinoid" & Retinol!!! I went straight in, applying nightly for 7 days before I realised it was the Retinol making my face sting. It took a full 3 weeks for the peeling and flaking to heal. I looked like I had painted my skin with PVA glue every day! :-(
