Probably a combination of people who held onto the packs and never opened them, knowing they would be worth money in the future, and old stock from stores that either somehow never sold / were hidden transferred or otherwise kept off shelves.

I'll never forget as a kid after helping my dad pressure was houses he had to go to home depot to buy more simple green spray. I spent some of my money on two packs. This was late 90s so gen 1(the ones with the yellow cardboard backs). I pulled a hollow zapdos and a hollow charizard that day. Was probably the best day of my childhood.
