Why might NEWS criteria go up or down.

God bless you! Thank you for taking the time to make these very informative videos. You're saving lives by doing these videos-you really are... So glad I found you! You've made me think about vitals in a different way, and that can only be good for my progression and the care of my patients. I'm a nursing student in my first year, and I want to be the best I can be for my level. Much thanks Doctor...

Thank you so much for posting .. You cant imagine how helpful your videos are, for 1st year student in nursing like me.. And I will be coming back throughout mine 2nd and 3rd year I guess.. Would you be doing lesson about A&P and possibly Waterlow, MUST, FALLS ,holistic care ,patient centre care. thank you

I’m so glad you mention that 210 systolic is high. Many nurses think this is normal as it does not score on the NEWS!

would you please clear the modification way in case the abnormal reading of specific vital sign is normal to our patient, for example a patient with normal heart rate 90 beat per min

Is high glucose (sugar) rather than high salt intake one of the triggers for hypertension ... if salt is taken out can someone with hypertension have problems with electrolytes if there is a high sugar /starch intake? What if salt intake is low and glucose/fructose is really low... should hypertension generally go away? You also mentioned ketones in relation to type one diabetic absent of insulin what about ketones in the case of hypertension with no type one diabetes?

Hi I just want to know if we have to indicate the values or just signify with a dot? Because the last video you put in the values with a dot while in this video you specified the value on the boxes with numericals

There have been methods at different times, the current practice is to put in the numbers. However, for the BP we also add top and bottom markers.

Please just everyone remember please please whether you are already a nurse or a student nurse or a nursing assistant or a doctor etc. Please please do not just treat a patient based on a score. Since this system was introduced it has worked well to escalate to a doctor.....but what I see more is the score being used and no other clinical judgement. I have seen NEWS0 written for hours upon hours on a chart, same written in patient notes, and have gone through the previous vital signs to find that the patient has actually been showing signs of decline hours earlier. Sometimes days! And multiple times. It is a score to escalate nothing more nothing less and whilst it is a useful tool.... Your patient could be scoring 0 for 48 hours and actually not be stable. You need to look at the trends....this is what saves lives......this is what prevents a crash call. Early intervention BEFORE a patient scores 5 or above! Just always remember USE YOUR CLINICAL JUDGEMENT. A NEWS of Zero does not indicate 'stable' vital signs. It is not the same thing. It does not indicate a stable patient.

Excellent point of course, however the RCP who developed this score seemed to think that good clinical judgement is a little thin on the ground. Defiantly a few research publications there is you fancy the challenge.

@@Campbellteaching I agree that over twenty one years I have seen some bad clinical judgement. However whilst a very useful tool, clinical judgement now has been fully removed from a lot of post news score implementation staff. They have been trained to use a score and intervene late. I am up for the challenge for sure. If it improves patient outcome. Always.
