Wouldn't low hemoglobin indicate need for oxygen since it carries oxygen? I picked hemoglobin of 7 for that question

I totally agree with you. the patient is in distress i dont think placing patient on low salt intake is a priority. Lasix would be administered to decrease the fluid overload thereby decreasing bp.

How many other people have viewed these questions? I almost got every question wrong but I get all of the Kaplan answers correct. Number 2) A high BP and confusion sounds like increased ICP, not someone who needs oxygen. Saying they need oxygen is an assumption and isn't supported by any facts. A high heart rate would indicate an oxygenation problem. I selected the saturated bandages because according to ABCs, that is a C and is not normal. Number 5) You are also adding to the question and making an assumption that someone who is saying they are going to cut the cord lines are looking for a weapon. This person exhibits delusions and seems schizophrenic, whereas the patient screaming down the hallway makes it very clear that they are a hazard to other patients and staff members without adding any assumptions. Number 6) Again, how does anyone reading this question get an assumption that the baby is going to have trouble regulating heat? It says the crib is against the wall. All babies cribs at the head are against a wall. Baby cribs have the heaters to regulate their temperature right about them. It wouldn't matter where the crib was placed unless it's a safety issue. I chose 4 because according to Maslows, physiological is first. Number 8) You're adding to the question again, and assuming that toddlers can climb what... 10 feet to the highest cabinet in a house? A toddler can get a safety cap off of a regular medication bottle much easier than trying to climb all the way up to your highest cabinet. Number 10) A hemoglobin level is drawn from the blood and is much more reliable than an 02 reading. Someones oxygen level can be low on a pulse ox if their fingers are cold. Hemoglobin is a blood level and much more accurate than a pulse ox.

Erin Dilley Doing this review made me lose faith in following your instruction. These questions all require assuming and adding into the question or taking away from the question. If you just read the question and the answers without any assumption, no one would get any of these questions right because they aren't properly worded.

Hi, I just wanted to thank you and let you know, that I failed the NCLEX in 10/13 and re-took it today. Based on the "Pearson Vue trick", I passed. I reviewed your videos last night for prioritizing questions because I really hadn't found any that were like the ones on the NCLEX in the last 3 months of studying. I purchased a 10 week subscription from ncsbn thinking that since they make the test, they would have the best practice questions. Wrong! They had very few prioritizing questions and they were nothing like the actual NCLEX questions!!. Also you were the only one who ever mentioned room assignments, which is a big subject on the NCLEX!!!. There were no practice questions for that topic in any of my study material. I used NCSBN, NLN and an NCLEX review book from Linda Silvestri. I learned more from your videos, thanks. I wish I had gone with ReMar!

My only complaint is # 8. It's great to keep them in original packing to identify what is causing the poisoning, but you can leave them sitting out for the child to get to them, even with child safety. I understand toddlers can climb, but are we to assume she's not leaving them in the original packaging as well... I dunno this one discouraged me.

For question number two, why wouldnt the answer be C? The s/s seem indicative of compartment syndrome which is an emergency.

I was told to not read into the answers, so why would we assume that the person who thinks that the CIA is telling him to cut the phone lines may Acquire a weapon? Client safety would be reasonable? right? Wouldn't it be better if we look at the running patient first? This is question number 5.

I think that a better rationalization would be that a patient that has "command hallucinations" e.g. having the CIA tell him things, would be most prone to injury because there are actual voices that tell him to do things. if you know what I mean

Nice! Didn't take your review and got a 7/10....ALMOST 8 because I was stuck haha BUT I do think the answer about the CIA psych patient rational should be different. I think the main priority of why to choose that in general is because it is a command hallucination type statement. Those are serious. other than that, great!

The answer that read to place patient on a low salt diet, is a distractor, at the end of the answer, it reads to give patient oxygen and this is the priority.

My daughter failed (or thinks she failed) the NCLEX for the 2nd time today. She said the computer shut off at 260 and that normally means you failed. She getting discouraged for sure since this is her 2nd time taking the NCLEX. She done the prep class from Kaplan, although it was the on-line version and studied for so many hours. What else can she do (outside the normal stuff) to finally beat this monster of a test? Thanks!!!

This doesn't mean she failed. Tell her to keep her head up. My friend went up to 280 and it shut off and she ended up passing it. When the questions go up that high, they are pretty much giving her a second chance during the test by making sure she knows it.

i cant understand why not give lasix to a pt who is clearly having shortness if breath, htn secondary to fluid overload? I dnt see why low salt intake is a priority. Hes breathing will be reduced once the fluid is relieved and at the same time fix the acidosis

Since RN's can't intubate, wouldn't it be wise to notify the doctor that a patient has pulled out their ET tube FIRST? While the doc is on their way, you can call out for another nurse to bring you the ET tube supplies. In HURST review, we learned "if you can only do ONE thing to fix the patient"....?!?!?!

I boiled it down to circumstances. what would you do, what's most important, what's most safe, abc, scope of practice and delegation. those who get all wrapped up in content get anxious by the amount of content out there.
