Excellent presentation. One minor correction in Huntington disease there is Caudate atrophy and the Inter-caudate diameter is increased (the distance between the two Caudate is increased) sometimes the ventral Horne of lateral ventricles called Boxing because they get enlarged like a box. Very good but the 2 antipsychotics for parkinsons or DLB are quetiapine & clozapine. DLB may not be able to tolerate antipsychotics at all (get massive hypotension) I read this in Journal of American medical Directors Association years ago. Risperdal is better than olanzapine but it does have a lot of anti-DA activity

Pimvanserin is a new medication that is approved for Parkinson’s psychosis and is a inverse agonist at 5HT2a. Previously, Seroquel was the preferred for Parkinson’s because of the high Ki in binding to Dopamine receptors. I would argue that Olanzapine would be the next best and not Risperdal using similar logic. Also, both Seroquel and Olanzapine have more intrinsic anti-cholinergic activity than Risperdal which helps with rigidity.
