There is theory that karma shows from lv 19 but i dont really believe it. I dunno how creator do tho

The way invincibility frames work is that with each level up, the frames decrease, leaving you more likely to take more damage. Sans gives you KR during the battle to add more difficulty. And since your level 19, at that point you have no invincibility frames, so Sans does one damage to balance the fight. In this battle, you still have INV frames (barely), so Sans does 4 damage and adds no KR to balance the fight.
