Idk if its been confirmed or not, but ive come up with a theory. Those woodlike humanoid creatures? Maybe theyre in Level 10. DarkStar is the good person, trying to help us; those creatures are the enemies. They are the prisoners. Let me explain. First, the leeches. Theres obviously a reason why theyre based on leeches, just like the other enemies. The Monkeys were murderers, represented by their sharp edges; Agatha is the personification of abused children dying too early, confused and fueled with rage; Gold Watchers are greedy people, literally becoming what they so desired; Duckies were liars, frauds, hiding behind a shell to protect themselves but harm others; Clown Gremlins are trolls, hurting others for the sake of amusement and patching their inner demons; Reaper Nurses were women who were taken advantage of by men, and lash out specifically at men, and use mens natural lustfulness to lure them in for the kill; The Joy Joy Gang is likely abused animals, confused from passing away for doing something they thought was right, and gaining human personalities pertaining to the character they became; Mama Bear is an abusive and manipulative mother, and her Teddies are the children hurt and deformed from her actions. What about the leeches? Well, i believe that they are representations of something that has recently come to light: a corrupt justice system, especially relating to the police. They are shown seriously hurting people for no justifiable reason other than amusement, especially in prisons, and possibly damaging the mental state of a prisoner permanently. They leech off the prisoners pain, they take advantage of them to chase us down while theyre doing whatever in the background, and only intervene when THEY are in trouble. They are manipulators, which is exactly what we needed to patch up the missing sins of humanity. I believe, based on this, that the leech characters ARE the bosses of this level, if not just minibosses before the prison warden. Now back to the original point: Theres no reason for the leeches to even BE leeches if they cant leech off something, right? Theyre leeching off the prisoners, obviously. But theres another layer to this. Theyre this deep into the game, these Rings of Hell, because theyre special. They were the ones who got away with their crimes. All the other enemies that were true sins were punished, allowing them free reign of their own worlds. The Joy Joy Gang and Nurses got their own safe spaces that were unlikely to be found by mortal souls, but if they were found they were either excessively violent or too trusting of those of the same gender (unless its the Gold Watchers, i guess theyre that obsessed with monetary value that women are nonexistent to them). However, because these souls were unchecked and unpunished, Hell forced them into an even worse nightmare they could never escape to pay for their actions. The prison guards became manipulative towards these prisoners, becoming leeches as the souls, that used to be like Doug's, become more and more deformed and more violent from the abuse they go through. Kinda sad to think about when you disregard why theyre even there in the first place.

yeah thats actually a decent theory, especially since a lot of the levels in DD do seem to be metaphors for corrupt institutions like the hospital, and deadly decadence level, which would fit very well with the levels being intertwined metaphors for various sins, the prison level further excentuating corruption and manipulation, which ties back into the selfishness and cruelty of doug's actions while he was alive

By the way, there aren't eleven levels, there's twelve. The last one being the s rank ending. Also vince confirmed that dark star isn't the boss of prison panic.

In Bearly Buried, we're introduced to a blue orb that helped Doug in that level and DarkStar has blue star irises the same color of blue as the blue orb. So..... thoughts?
