Broadway is a section of town in New York with very select theatres which are considered the cream of the crop. Off-Broadway runs are usually done to perfect a show with several drafts and touchups.

Yes, it's a flashback to when he and Diana were in college. He is comparing the present Diana to the one back then. She, and it was said to be due to her bi-polar disorder, was wild, exciting, and somewhat reckless (she wanted to cut class with him again). This is also what caused her to plan the trip for "helicopter skiing! :D" and got her so excited about cake. Now on the other hand, she's depressed, and he has to keep the family together, no longer feeling the exciting love he used to.

Dan is dan. The doctors on broadway are the same actor. The doctors in this one is played by Asa Somers.

This is before it was on broadway so that's why some stuff is different like there's no "Everything Else" so this is the pre-broadway show which also explains why Dan and the Dr. Are different people. Hope that helps :)
